Étude de phase II portant sur l’anticorps anti-PD-L1 atézolizumab, le bévacizumab et l’acide acétylsalicylique visant à évaluer la sécurité d’emploi et l’efficacité de cette association chez des patientes avec un adénocarcinome récidivant et résistant au platine de l’ovaire, des trompes de Fallope ou péritonéal primitif.

Zusammenfassende Beschreibung der Studie (Datenquelle: BASEC)

Cette étude de phase II a pour but de tester un nouveau médicament pas encore autorisé en Suisse, administré en association avec un traitement connu ou seul. L'objectif est de déterminer si le médicament atézolizumab, et les associations de l’atézolizumab avec le bévacizumab et/ou l’acide acétylsalicylique, montrent une activité contre le cancer des ovaires et/ou sont sans danger. L’atézolizumab est un médicament «d’immunothérapie», qui stimule le système immunitaire de l'organisme de manière à ralentir ou arrêter la croissance tumorale en bloquant PD-L1 (Programmed Death-Ligand 1). Le bévacizumab, déjà approuvé pour le traitement de cette maladie, est un anticorps qui inhibe la formation de vaisseaux dans la tumeur en bloquant le facteur de croissance endothélial vasculaire (Vascular Endothelial Growth Factor (VEGF)). Comme certaines données laissent penser que l’acide acétylsalicylique (aspirine ou AAS) pourrait contribuer à renforcer l’efficacité de ces médicaments, nous étudions également l’ajout d’aspirine à ces médicaments. Cet essai teste la valeur de l’association d’un médicament anticancéreux immunologique, l’atézolizumab, avec le bévacizumab et/ou l’aspirine. Update protocol v5.0: Les groupes 2 et 3 ont étés fermés prématurément en raison de la possibilité que la thérapie avec l’atezolizumab seul, avec l’aspirine/placebo, ne soit pas suffisamment efficace pour traiter votre cancer par rapport aux 3 autres traitements.

Kriterien zur Teilnahme an der Studie (Datenquelle: BASEC)

- Cancer de l'ovaire (épithélial, des trompes de Fallope, péritonéal primaire) récurrent, confirmé histologiquement, résistant au platine, de stade avancé ou métastatique
- Au moins une lésion accessible à la biopsie sans mise en danger du patient
- Traitement préalable systémique (chimiothérapie, hormonothérapie, agents biologiques et/ou vaccins) ou radiothérapie.

Ausschlusskriterien (Datenquelle: BASEC)

- Participation actuelle, récente (dans les 4 semaines avant la randomisation) ou planifiée à une étude expérimentale de médicament
- Hypercalcémie incontrôlée ou symptomatique nécessitant un traitement continu aux biphosphonates ou au dénosumab.
- Maladie symptomatique du cerveau ou leptoméningée; toute métastase cérébrale ne progressant pas depuis au moins 6 moins.

Inclusion/Exclusion Criteria (Datenquelle: WHO)


Inclusion Criteria:

Recurrent, histologically proven, platinum-resistant, epithelial ovarian, fallopian tube
and primary peritoneal cancer in advanced or metastatic stage. Histological diagnosis by
image guided biopsy, laparoscopy or laparotomy. Tumors diagnosed on cytology only and
borderline tumors are excluded.

At least one lesion accessible to biopsy without putting patient at risk

WHO PS: 0-2 for patients having received no more than two previous lines of therapy. WHO
PS: 0-1 for patients having received >2 previous lines of therapy

Prior chemotherapy or bevacizumab:

Any number of platinum-based chemotherapy lines are allowed but a maximum of 2 previous
non-platinum containing lines

Prior treatment with bevacizumab or other targeted agents against Vascular Endothelial
Growth Factor (VEGF) or VEGF receptor is allowed, but at least 18 weeks must have elapsed
since their last administration

Eligible patients with = 2 previous treatment lines must have been previously exposed to
bevacizumab or other targeted agents against VEGF or VEGF receptor

Patients may have had prior therapy providing the following conditions are met:

Radiation therapy: wash-out period of 14 days prior to the first study treatment;
exception: single fraction radiotherapy with the indication of pain control

Systemic anti-tumor therapy: wash-out period of 21 days prior to the first study treatment

Recovery from any toxic effects of prior therapy to = Grade 1 per the National Cancer
Institute Common Terminology Criteria for Adverse Events (NCI CTCAE v4.03) except fatigue
or alopecia.

Exclusion criteria:

Age <18 years

Life expectancy of < 12 weeks

No adequate hematologic and end organ function

Use of acetylsalicylic acid, NSAIDs or other COX-2 inhibitors that cannot be stopped at
baseline and for the whole duration of the study.
