Bei der Colitis ulcerosa (Cu) handelt es sich um eine chronisch-entzündliche Darmerkrankung, die zu Entzündungen und Blutungen an der Rektum- und Dickdarmschleimhaut führt. Es sind unterschiedliche Therapien zur Behandlung der Cu auf dem Markt erhältlich, aber sie sind nicht immer wirksam. In dieser Studie soll untersucht werden, ob ein neues Prüfpräparat (ABT 494) Patienten mit mittelschwerer bis schwerer aktiver Cu helfen kann. Die Studie besteht aus drei Teilstudien: Ziel der Teilstudie 1 (Phase 2b, Induktionstherapie) ist die Charakterisierung der Dosis-Wirkungs-Beziehung, Wirksamkeit und Sicherheit von ABT 494 im Vergleich zu Placebo im Hinblick auf die Einleitung einer klinischen Remission um die Induktionsdosis von ABT 494 zur weiteren Bewertung in Teilstudie 2 zu ermitteln. Ziel der Teilstudie 2 (Phase 3, Induktionstherapie) ist die Bewertung der Wirksamkeit und Sicherheit von ABT 494 (die Dosis die in Teilstudie 1 ermittelt wurde) im Vergleich zu Placebo im Hinblick auf die Einleitung einer klinischen Remission. Ziel der Teilstudie 3 (Phase 3, Erhaltungstherapie) ist die Bewertung der Wirksamkeit und Sicherheit von ABT 494 im Vergleich zu Placebo in der Erzielung einer klinischen Remission bei Patienten die auf eine Induktionstherapie mit ABT 494 angesprochen haben.

Die Studie besteht aus drei Teilstudien: einer Induktionsphase (Teilstudie 1 und 2) und einer anschließenden Erhaltungsphase (Teilstudie 3). Es werden insgesamt 1.055 Patienten an ca. 475 Prüfzentren weltweit teilnehmen.
Die Teilstudie 1 umfasst 8 Wochen und besteht aus 2 Phasen. In der ersten Phase wird die Wirksamkeit und Sicherheit von ABT 494 im Vergleich zu Placebo untersucht und die ABT 494-Dosis ermittelt, die in Teilstudie 2 angewendet werden soll. 250 geeignete Patienten werden in eine von fünf Behandlungsarmen randomisiert: vier beinhalten die Behandlung mit ABT 494 und einer mit Placebo. Niemandem ist die Dosis, die der jeweilige Patient erhält, bekannt (verblindete Studie). Nach Abschluss der Rekrutierung werden zusätzlich 100 Patienten randomisiert und erhalten verblindet ABT 494.
Die Teilstudie 2 umfasst 8 Wochen und besteht ebenfalls aus 2 Phasen. In der ersten Phase werden die Patienten im Verhältnis 2:1 entweder zu ABT 494 (Dosis auf Basis der Entscheidung in Teilstudie 1) oder Placebo randomisiert. Die Zuweisung erfolgt verblindet. An Phase 1 schließt sich Phase 2 an: In dieser erhalten 330 weitere geeignete Patienten ABT 494 (alle erhalten dieselbe Dosis). Die zweite Phase ist offen, d. h. die Patienten wissen, dass sie ABT-494 erhalten.
Teilstudie 3 umfasst eine 44 wöchige Erhaltungsphase für Patienten, die die Teilstudien 1 bzw. 2 abgeschlossen und auf ihre Behandlung angesprochen haben. Sie erhalten weiterhin dasselbe Prüfpräparat oder werden erneut entweder zu einer anderen Dosis oder zu Placebo randomisiert, je nachdem, was sie zuvor angewendet haben. Patienten, die die 8 wöchige Induktionsphase abgeschlossen, aber entweder auf die Behandlung nicht angesprochen, das Ansprechen in der Erhaltungsphase verloren oder diese Studie abgeschlossen haben, können an einer Fortsetzungsstudie teilnehmen, in der sie ABT 494 erhalten.
Während den Visiten am Studienzentrum werden verschiedene Untersuchungen (wie z.B. Endoskopie) durchgeführt sowie Blut-, Urin-, Gewebe- und Stuhlproben entnommen.

Criteria for participation in trial (Data source: BASEC)

- Männer oder Frauen, die zwischen 18 und 75 Jahre alt sind und eine Diagnose einer Colitis ulcerosa mindestens 90 Tage vor Studienbeginn haben.
- Aktive Colitis ulcerosa mit angepasstem Mayo-Score von 5 bis 9 Punkten und Teilscore hinsichtlich Endoskopie von 2 bis 3.
- Nachweislich unangemessenes Ansprechen oder Verlust des Ansprechens auf oder Unverträglichkeit gegenüber Kortikosteroiden, Immunsuppressiva und/oder Biologika.

Exclusion criteria (Data source: BASEC)

- Aktuelle Diagnose eines Morbus Crohn oder Diagnose einer Colitis indeterminata.
- Aktuelle Diagnose einer fulminanten Colitis und/oder eines toxischen Megakolons.
- Vorherige Behandlung mit einem JAK Inhibitor (z. B. Tofacitinib, Baricitinib, Filgotinib).

Inclusion Criteria:

Note: Adolescent participants at the age of 16 or 17 years old will be eligible to
participate if approved by the country or regulatory/health authority. If approval has not
been granted, only participants >= 18 years old will be enrolled.

Adolescents must weigh >= 40 kilograms and meet the definition of Tanner Stage 5 at
Screening Visit.

- Diagnosis of ulcerative colitis for 90 days or greater prior to Baseline, confirmed by
colonoscopy during the Screening Period, with exclusion of current infection, colonic
dysplasia and/or malignancy. Appropriate documentation of biopsy results consistent
with the diagnosis of UC, in the assessment of the Investigator, must be available.

- Active ulcerative colitis with an Adapted Mayo score of 5 to 9 points and endoscopic
sub score of 2 to 3 (confirmed by central reader).

- Demonstrated an inadequate response to, loss of response to, or intolerance to at
least one of the following treatments including: oral aminosalicylates,
corticosteroids, immunosuppressants, and/or biologic therapies in the opinion of the
investigator.

Note: Participants who have had inadequate response, loss of response to conventional
therapy, but have not failed biologic therapy (Non-bio-IR) and have received a prior
biologic for up to 1 year may be enrolled, however they must have discontinued the biologic
for reasons other than inadequate response or intolerance (e.g., change of insurance, well
controlled disease) and must meet criteria for inadequate response, loss of response or
intolerance to aminosalicylates, corticosteroids, and/or immunosuppressants as defined
above.

- If female, participant must meet the criteria for Contraception Recommendations.

- Female participants of childbearing potential must have a negative serum pregnancy
test at the Screening Visit and a negative urine pregnancy test at the Baseline Visit
prior to study drug dosing.

Exclusion Criteria:

- Participant with current diagnosis of Crohn's disease (CD) or diagnosis of
indeterminate colitis (IC).

- Current diagnosis of fulminant colitis and/or toxic megacolon.

- Participant with disease limited to the rectum (ulcerative proctitis) during the
Screening endoscopy.

- Received cyclosporine, tacrolimus, mycophenolate mofetil, or thalidomide within 30
days prior to Baseline.

- Participant on azathioprine or 6-mercaptopurine within 10 days of baseline.

- Received intravenous corticosteroids within 14 days prior to Screening or during the
Screening Period.

- Participant with previous exposure to Janus Activated Kinase (JAK) inhibitor (e.g.,
tofacitinib, baricitinib, filgotinib, upadacitinib).

- Screening laboratory and other analyses show any abnormal results.
