Brief description of trial (Data source: BASEC)
Bei einer Hirnblutung kommt es zu permanenten Schäden im Gehirn, welche eine bleibende (schwere) Behinderung nach sich ziehen können. Mit der Zeit kann die Blutung auch zunehmen, was bedeutet, dass der Grad der Behinderung grösser werden oder dies gar zum Tod führen kann. Dies ist insbesondere bei Patienten, welche eine Blutverdünnung (NOAC) nehmen, der Fall. Zum jetzigen Zeitpunkt gibt es keine geprüfte Behandlung, welche erwiesenermaßen eine Blutung im Gehirn stoppen und zur Verbesserung der Symptome führen kann. In dieser Studie testen wir, ob es möglich ist, Patienten mit einer Hirnblutung innerhalb der ersten Stunden nach Symptombeginn mit einem Medikament – Tranexamsäure – zu behandeln. Wir möchten zeigen, dass eine Behandlung mit diesem Medikament die Rate an Nachblutungen reduziert.Die Studienmedikation ist eine Substanz, welche dazu führt, dass das Blut verklumpt und eine Blutung gestoppt wird. Dieses Medikament wird nicht in der Routine zur Behandlung einer Hirnblutung eingesetz. Tranexamsäure ist ein etabliertes Medikament, das bereits seit Jahren eingesetzt wird und zuverlässig und schnell Blutungen stoppen kann. Um eine klare Aussage über die Wirkung von Tranexamsäure bei Patienten mit Hirnblutungen unter NOAC machen zu können müssen wir im Rahmen dieser Studie manchen Patienten das Medikament verabreichen und manchen ein sogenanntes Placebo – eine gleich aussehende Lösung, welche jedoch keine Tranexamsäure enthält.

1000 mg Tranexamsäure in 100 ml NaCl 0.9% werden intravenös innerhalb 10 Minuten appliziert. Es folgt eine Infusion mit 1000 mg Tranexamsäure in 250 ml NaCl 0.9% über die Dauer von 8 Stunden.

Criteria for participation in trial

- akute, innerhalb der letzten 12 Stunden aufgetretene Hirnblutung
- Patienten stehen unter Behandlung mit einem neuen oralen Antikoagulanz, das zuletzt innerhalb der letzten 48 Stunden eingenommen worden ist (Apixaban, Dabigatran, Edoxaban oder Rivaroxaban)
- Alter>18 Jahre
- Einverständniserklärung vorhanden

- Sekundäre intrazerebrale Blutung in Folge arteriovenöser Missbildung, Tumor, Trauma ect.
- Schwangerschaft

Inclusion Criteria:
- Acute intracerebral hemorrhage (symptom onset <12h)
- Prior treatment with a novel direct oral anticoagulant (apixaban, dabigatran, edoxaban
or rivaroxaban; last intake <48hours or proven NOAC activity by relevant coagulation
assays)
- Age >18 years, No upper age limit
- Informed consent has been received in accordance to local ethics committee
requirements
Exclusion Criteria:
- Severe pre-morbid disability (modified Rankin scale >4)
- Anticoagulation with Vitamin K antagonists (VKA) (recent intake)
- Secondary intracerebral hemorrhage (e.g. arteriovenous malformation (AVM), tumor,
trauma) Note it is not necessary for investigators to exclude underlying structural
abnormality prior to enrolment, but where an underlying structural abnormality is
already known, these patients should not be recruited.
- Glasgow coma scale <5
- pregnancy
- Planned neurosurgical hematoma evacuation within 24 hours (before follow-up imaging)
- Pulmonary embolism/deep vein thrombosis within the last 2 weeks.
