Eine internationale multizentrische, offene Studie zur Bewertung der Sicherheit, Verträglichkeit, Biodistribution, Dosimetrie und der vorläufigen Wirksamkeit von 177Lu-OPS201 für die Therapie Somatostatin-Rezeptor-positiver neuroendokriner Tumore (NETs).

Brief description of trial (Data source: BASEC)

Es sollen insgesamt bis zu 55 Patienten mit 177Lu-OPS201 behandelt werden. Im ersten Teil (Teil A) der Studie werden bis zu 15 Patienten mit jeweils 3 Zyklen 177Lu-OPS201 (300 ± 50 µg Peptid/Zyklus) mit einer Radioaktivität von 4,5 GBq (Giga-Bequerel) im Abstand von jeweils 8 Wochen behandelt. Nachdem die ersten 6 Patienten in Teil A der Studie 3 Behandlungszyklen abgeschlossen hatten, bewertete ein Ausschuss von Ärzten und medizinischen Experten die sicherheitsrelevanten Daten, also mögliche Nebenwirkungen, und die Strahlenbelastung wichtiger Organe, und entschied, den zweiten Teil (Teil B) der Studie mit einer höheren Peptidmassendosis und/oder Radioaktivität zu beginnen. Im zweiten Teil (Teil B) der Studie werden bis zu 40 Patienten in bis zu 8 Kohorten (Behandlungsgruppen) mit 177Lu-OPS201 behandelt. In jeder Kohorte erhält jeder Patient 3 Zyklen der von einem Ausschuss von Ärzten und medizinischen Experten festgelegten Radioaktivitäts- und Peptidmassendosis von 177Lu-OPS201 im Abstand von jeweils 8 Wochen. In Abhängigkeit von dem klinischen Nutzen und der Verträglichkeit können die Patienten von Teil B bis zu 2 zusätzliche Gaben von 177Lu-OPS201 erhalten. Nach jedem Behandlungszyklus wird die Strahlenbelastung der inneren Organe überprüft. Sollte die Strahlenbelastung bestimmte Werte überschreiten, wird die Aktivität des jeweils nächsten Zyklus angepasst. Im Anschluss an eine Abschlussvisite nach dem 3. Behandlungszyklus und zudem - falls zutreffend - nach den bis zu zwei möglichen zusätzlichen Behandlungszyklen finden über einen Zeitraum von 2 Jahren Langzeit-Nachbeobachtungsvisiten im Abstand von jeweils 3 Monaten statt, um den Verlauf der Erkrankung und die Sicherheit der Studienteilnehmer weiter zu beobachten.

Gastroenteropankreatischer neuroendokriner Tumor (GEP NET), Lungen-NET, Phäochromozytom oder Paragangliom. Die Zellen dieser Tumore tragen eine erhöhte Anzahl bestimmter Eiweissmoleküle (sog. Somatostatinrezeptoren) auf der Zelloberfläche. Diese Tumore sind daher gut geeignet, um die Sicherheit und Wirksamkeit von 177Lu-OPS201 (177Lu-Satoreotid-Tetraxetan) zu untersuchen, da 177Lu-OPS201 sehr gut an diese Somatostatinrezeptoren bindet.

Bei der in dieser Studie untersuchten Intervention (Eingriff) handelt es sich um eine Peptid-Rezeptor-Radionuklidtherapie (PRRT) mit 177Lu-OPS201.
177Lu-OPS201 ist ein in der Erprobung befindliches Medikament, das zurzeit in klinischer Entwicklung ist, das bedeutet, dass noch nicht bekannt ist, ob dieses Medikament Patienten hilft, und dies heisst, dass es für die Behandlung für Somatostatin-Rezeptor-positive NETs (neuroendokrine Tumore) nicht zugelassen ist. 177Lu-OPS201 besteht aus drei Teilen: dem radioaktiven Teil (oder Isotop) Lutetium-177 (177Lu), welches über eine weitere Substanz (DOTA) mit einer kurzen Aminosäure (einem Peptid) verbunden ist. Dieser Peptid-Anteil bindet sich an bestimmte Strukturen auf der Zelloberfläche, die als Somatostatinrezeptoren bezeichnet werden. Neuroendokrine Tumoren (NET) weisen diese Rezeptoren auf ihrer Oberfläche auf. Durch die Bindung des Peptidanteils an die Rezeptoren wird die radioaktive Substanz (Lutetium-177) in die unmittelbare Nähe des Tumors gebracht und kann so mittels seiner radioaktiven Strahlung den Tumor bekämpfen. Diese Art der Therapie wird allgemein als Peptidrezeptor-Radionuklidtherapie (PRRT) bezeichnet. Sie hat vor einigen Jahren Eingang in die Krebsbehandlung gefunden und wird, je nach Art der Tumore, mit verschiedenen Peptiden und radioaktiven Substanzen durchgeführt.
Im Vergleich zu anderen Substanzen, die an Somatostatinrezeptoren binden können, ist die Bindung von 177Lu-OPS201 besonders stark und spezifisch, wodurch eine bessere Wirksamkeit gegen den Tumor vermutet wird. Hinweise darauf haben Labor- und Tierversuche ergeben.
Der radioaktive Anteil (Lutetium-177) hat eine physikalische Halbwertszeit von 6,65 Tagen. Die physikalische Halbwertszeit ist hierbei die Zeitspanne, in welcher sich die Menge und Aktivität des radioaktiven Elements 177Lu durch den Zerfall halbiert hat. Die freigesetzte Strahlung bewegt sich nur eine sehr kurze Strecke in das Körpergewebe hinein (im Mittelwert 0,23 Millimeter, maximal 1,7 Millimeter), so dass durch die enge Nachbarschaft der Strahlungsquelle zum Tumor vor allem das Tumorgewebe geschädigt und umliegendes gesundes Gewebe grösstenteils geschont wird.
177Lu-OPS201 wurde bislang bei mehr als 30 Patienten mit NETs in klinischen Studien angewendet.

In dieser Studie sollen die Sicherheit und Verträglichkeit der Behandlung mit 177Lu-OPS201 bei Patienten mit Somatostatin-Rezeptor-positiven neuroendokrinen Tumoren (NETs) untersucht werden.
Ausserdem soll die Verteilung von 177Lu-OPS201 im Körper (Biodistribution), die Geschwindigkeit der Entfernung von 177Lu-OPS201 aus dem Körper (Pharmakokinetik), die Reaktion des Immunsystems auf die Behandlung mit 177Lu-OPS201 (systemische Immunantwort), die Verteilung der abgegebenen Strahlung im Körper (Dosimetrie) und die vorläufige Wirksamkeit der Behandlung mit 177Lu-OPS201 bei Patienten mit Somatostatin-Rezeptor-positiven neuroendokrinen Tumoren (NETs) untersucht werden.
Das Ziel ist die Bewertung der optimalen Radioaktivität und Peptidmassendosen für die zukünftige Therapie und die Bewertung des Ansprechens auf die Behandlung durch die Bestimmung von Veränderungen des Tumorvolumens.

1) Histologisch bestätigte Diagnose:
- eines inoperablem GEP NET (Grad I und Grad II entsprechend WHO-Klassifizierung)
- eines inoperablen „typischen Lungenkarzinoids“ oder „atypischen Lungenkarzinoids“ ist akzeptabel (mit Ausnahme von grosszelligen neuroendokrinen Neoplasien der Bronchien und kleinzelligen
Lungenkarzinomen)
- eines malignen, inoperablen Phäochromozytoms oder Paraganglioms

2) Unter vorheriger Behandlung fortschreitende Erkrankung (Progressive Disease) basierend auf RECIST Kriterien (RECIST Version 1.1.) innerhalb von 6 Monaten vor Studieneinschluss (allerdings kann die Progression auch mehr als 6 Monate vor dem Studieneinschluss aufgetreten sein). Keine weitere Tumorbehandlung der Patienten nach der Dokumentation der fortschreitenden Erkrankung (Krankheitsprogression)

3) Bestätigtes Vorliegen von Somatostatinrezeptoren auf technisch auswertbaren Tumorläsionen; dokumentiert durch einen positiven Somatostatinrezeptor-Scan innerhalb von 6 Monaten vor Einschluss des Patienten in die Studie

Exclusion criteria (Data source: BASEC)

1) Jede vorherige Peptid-Rezeptor Radionuklid-Therapie (PRRT); Teil B: Nephrektomie (chirurgische Entfernung einer Niere) oder begleitende nephrotoxische Behandlung, die den Patienten nach dem Urteil des Prüfarztes während der Studie einem hohen Risiko für Nierentoxizität aussetzt

2) Jede ausführliche Strahlentherapie ≤ 3 Monat vor Einschluss

3) Chemotherapie ≤ 3 Wochen vor Einschluss

Inclusion Criteria:

1. Written informed consent.

2. Patients of either gender, aged = 18 years.

3. Women of childbearing potential (not surgically sterile or less than 2 years
postmenopausal) must use a medically accepted method of contraception and must agree
to continue use of this method for the duration of the study and for 6 months after
the last dose. Acceptable methods of contraception include abstinence, or double
contraception: steroidal contraceptive (oral, transdermal, implanted, and injected) in
conjunction with a barrier method (intrauterine device, condom etc.).

4. Male patients must use a medically accepted method of contraception and must agree to
continue use of this method for the duration of the study and for 6 months after the
last activity administration.

5. Karnofsky performance score = 60.

6. Life expectancy of at least 6 months.

7. Histologically confirmed diagnosis of -

- unresectable GEP NET (Grade I and Grade II according to WHO classification (2010,
Annex 01), functioning and non-functioning).

- unresectable "typical lung carcinoid" or "atypical lung carcinoid" are acceptable
(with the exception of Large Cell Bronchial Neuroendocrine Neoplasms and Small
Cell Lung Cancers) (Caplin 2015).

- malignant, unresectable pheochromocytoma or paraganglioma

8. Documentation of progressive disease based on RECIST v1.1 under prior anti-tumor
therapy within 6 months of entry in the study (although the progression might have
occurred more than 6 months before study entry). Patients should not have received
further anti-tumor therapy once disease progression is documented. The images of this
evaluation should be available for TGR evaluation.

9. In countries where sunitinib or everolimus are marketed, patients with GEP NET and
lung NET will be progressive under this prior anti-tumor treatment for the respective
indication. Patients not suitable for everolimus/sunitinib therapy according to a
tumor board decision (or comparable local practice) may also be enrolled into the
study. Patients having everolimus/sunitinib therapy should have a wash-out phase of =
4 weeks before the first treatment.

10. Measurable disease based on RECIST v1.1.

11. Confirmed presence of somatostatin receptors on technically evaluable tumor lesions
documented by a positive Somatostatin Receptor Scan performed within 6 months prior to
enrolment in the study.

12. Calculated GFR = 55 mL/min.

13. Blood test results as follows:

- Leukocytes: = 4*10^9/L

- Erythrocytes: = 3.5*12^9/L

- Platelets: = 100*10^9/L

- Albumin: > 30 g/L

- ALT, AST, AP: = 5 times ULN (upper limit of normal)

- Bilirubin: = 2 times ULN (2x 1.1 mg/dL)

Exclusion Criteria:

1. Known hypersensitivity to 177Lu, to DOTA, to JR11 or to any of the excipients of
177Lu-OPS201.

2. Any previous peptide receptor radionuclide therapy (PRRT).

3. Diagnosis of thymic NET.

4. Presence of active infection at screening or history of serious infection within the
previous 6 weeks.

5. Administration of any other investigational medicinal product within 60 days prior to
entry.

6. Prior or planned administration of a therapeutic radiopharmaceutical within 8
half-lives of the radionuclide including any time during the current study.

7. Any extensive radiotherapy = 3 months before enrolment.

8. Chemotherapy = 3 months before enrolment.

9. Nephrectomy, renal transplant or concomitant nephrotoxic therapy putting the subject
at high risk of renal toxicity during the study as assessed by the investigator.

10. Pregnant or breast-feeding women: A pregnancy test will be performed at the start of
the study for all female patients of childbearing potential (i.e. not surgically
sterile or up to 2 years postmenopausal).

11. Any uncontrolled significant medical, psychiatric or surgical condition (active
infection, unstable angina pectoris, cardiac arrhythmia, poorly controlled
hypertension, poorly controlled diabetes mellitus [HbA1c =9%], uncontrolled congestive
heart disease, etc.) or laboratory findings that, in the opinion of the investigator,
might jeopardize the patient's safety or that would limit compliance with the
objectives and assessments of the study. Note: the patient should be able to tolerate
high volume load.

12. Current history of any malignancy other than NET within 5 years of enrolment except
for fully -resected non-melanoma skin cancer or cervical cancer in situ. Current
history of malignancy; patients with a secondary tumor in remission of > 5 years can
be included

13. Any mental condition rendering the patient unable to understand the nature, scope and
possible consequences of the study, and/or evidence of an uncooperative attitude.
