Brief description of trial (Data source: BASEC)

La raison de cette étude est pour évaluer l’efficacité (si le médicament fonctionne bien) de Venetoclax chez les patients atteints ayant récidivé (c’est-à-dire que votre cancer est revenu) ou réfractaire (votre cancer n’a pas répondu aux traitements anti-cancéreux mis en place jusqu’ici) d’une Leucémie lymphoïde chronique (LCC), y compris à des patients dont le chromosome 17 n’est peut-être pas complet c’est-à-dire chez lesquels une délétion 17p ou une mutation du gène TP53 ont été observées — ou encore – des patients qui ne répondent plus ou ne tolèrent plus un traitement avec des inhibiteurs du récepteur des cellules B (BCRi).

Tous les patients recevront unde dose de départ de 20 mg par jour. Cette dose sera augmentée graduellement jusqu’à 400 mg par jour au cours des 5 semaines suivantes. Les patients prendront venetoclax pendant 108 semaines. Ils seront ensuite suivis pendant 2 ans. Les effets du traitement sur le LLC seront contrôlés par des prises de sang, par des examens physiques, un CT-scan au début and à la semaine 48, en collectant less effets indésirables et en complétant des questionnaires de santé.

Drug: Venetoclax

Criteria for participation in trial (Data source: BASEC)

1. Âge ≥ 18 ans.
2. Score de performance ≤ 2 à l’Eastern Cooperative Oncology Group (ECOG).
3. être atteint d’un cancer récidivant/réfractaire (doit avoir suivi déjà au moins un traitement).
4. Diagnostic de LLC qui satisfait aux critères de l’IWCLL NCI-WG Guidelines modifiés de 2008:
• Avoir une indication de traitement conformément aux critères de l’IWCLL NCI-WG modifiés de 2008
• Avoir un cancer cliniquement mesurable (lymphocytose > 5 × 109/L et/ou nodules palpables et mesurables à l’examen physique et/ou organomégalie évaluée à l’examen physique)
De plus, les patients:
• Peuvent avoir une délétion de 17p ou une mutation de TP53 dans la moelle osseuse ou le sang périphérique, évaluées par un laboratoire local ET/OU
• Peuvent avoir été précédemment traités avec un inhibiteur des récepteurs des cellules B, même si ce traitement a échoué ou si le patient s’est montré intolérant à ce traitement (p. ex. toxicité).
5. Fonction médullaire adéquate définie comme suit:
• plaquettes ≥ 25,000/mm3 sans aucune des situations suivantes:
support transfusionnel dans les 14 jours précédant la sélection
preuve de saignement des muqueuses
antécédents connus d’épisode de saignement majeur dans les 3 mois précédant la sélection
• hémoglobine ≥ 8,0 g/dL

Exclusion criteria (Data source: BASEC)

1. le sujet a développé un syndrome de Richter ou une leucémie prolymphocytaire (LPP)
2. le sujet a déjà reçu du venetoclax.
3. le sujet a des antécédents de tumeurs actives autres qu’une LLC dans les 2 ans précédant l’administration de la première dose de venetoclax, à l’exception de:
• un carcinome in situ du col de l’utérus traité adéquatement
• un carcinome basocellulaire ou un carcinome spinocellulaire localisé de la peau
• une ancienne tumeur confinée et chirurgicalement réséquée (ou traitée selon d’autres modalités) avec intention curative.
4. Cytopénies auto-immunes actives et non contrôlées (dans les 2 semaines précédant la sélection), y compris une anémie hémolytique auto-immune (AHAI) ou un purpura thrombocytopénique idiopathique (PTI), malgré de faibles doses de corticostéroïdes.
5. le sujet a été traité par une greffe allogénique de cellules souches

Inclusion/Exclusion Criteria (Data source: WHO)


Inclusion Criteria:

- Participant has Eastern Cooperative Oncology Group (ECOG) performance score of less
than or equal to 2

- Participant has relapsed/refractory disease (received at least 1 prior therapy)

- Participant has diagnosis of CLL that meets published 2008 Modified International
Workshop for Chronic Lymphocytic Leukemia National Cancer Institute Working Group
(IWCLL NCI-WG) Guidelines and:

- has an indication for treatment according to the 2008 Modified IWCLL NCI-WG
criteria

- has clinically measurable disease (lymphocytosis greater than 5 × 10^9/L and/or
palpable and measurable nodes by physical exam and/or organomegaly assessed by
physical exam)

- In addition, participants:

- with or without 17p deletion or TP53 mutation, assessed by a local laboratory in
bone marrow or peripheral blood are eligible

- may have been previously treated with a prior B-cell receptor inhibitor

- Participant must have adequate bone marrow function, coagulation profile, renal, and
hepatic function, per laboratory at Screening

Exclusion Criteria:

- Participant has developed Richter's transformation or Prolymphocytic leukemia

- Participant has previously received venetoclax

- History of active malignancies other than CLL within the past 2 years prior to first
dose of venetoclax, with the exception of:

- adequately treated in situ carcinoma of the cervix uteri

- adequately treated basal cell carcinoma or localized squamous cell carcinoma of
the skin

- previous malignancy confined and surgically resected (or treated with other
modalities) with curative intent

- Participant has active and uncontrolled autoimmune cytopenias (for 2 weeks prior to
Screening), including autoimmune hemolytic anemia and idiopathic thrombocytopenic
purpura despite low dose corticosteroids

- Participant has undergone an allogeneic stem cell transplant

- Treatment with any of the following within five half-lives or 14 days (if half-life
unknown) as applicable prior to the first dose of venetoclax, or clinically
significant adverse effect(s)/toxicity(s) of the previous therapy have not resolved to
< National Cancer Institute Common Terminology Criteria for Adverse Events (NCI CTCAE)
v4.03 Grade 2:

- Any anti-cancer therapy including chemotherapy, or radiotherapy;

- Investigational therapy, including targeted small molecule agents

- Participant is human immunodeficiency virus (HIV) positive

- Participant has known allergy to both xanthine oxidase inhibitors and rasburicase
