Brief description of trial (Data source: BASEC)

La chirurgie du cancer du sein est une chirurgie qui comporte un risque élevé des douleurs chroniques postopératoires. Cependant, il y a des facteurs qui augmentent le risque personnel d’une personne : la douleur au site chirurgical avant la chirurgie, l’antécédent d’une dépression, l’âge de moins de 50 ans, et la crainte des douleurs fortes postopératoires. Avec ces facteurs, on peut calculer un score de risque afin de prédire le risque. L’étude cible les patientes avec un risque haut de plus de 30% de développer une douleur chronique. L’étude est « contrôlée par placebo », c’est-à-dire compare un groupe de patientes qui va recevoir le médicament avec un autre qui va recevoir une substance inactive (placebo). Elle est menée « randomisée », c’est-à-dire que l’attribution d’un patient aux deux groupes se fait au hasard, et « à double aveugle », c’est-à-dire que ni le patient, ni les chercheurs vont savoir avant la fin de l’étude à lequel des deux groupes un patient a été attribué. Le but de l’étude est de montrer que la prise du médicament prégabaline pendant 2 semaines après la chirurgie va diminuer le risque d’avoir des douleurs 6 et 12 mois après. Un objectif additionnel est de récolter des échantillons de cellules par des frottis oraux afin d’investiguer des liens potentiels entre la disposition génétique et la survenue des douleurs persistantes.

Le médicament avec lequel l’étude teste de prévenir ce douleur est la prégabaline, utilisé déjà depuis plusieurs années pour traiter des douleurs chroniques. D’autres études ont montré qu’il faut prendre ce médicament pour au moins 2 semaines après la chirurgie et commencer la veille de l’opération. Les effets indésirables de ce médicament, qui ne sont pas dangereux, mais parfois gênants, sont également bien connus. Il s’agit surtout des étourdissements et d’une somnolence.

- femmes qui vont avoir une chirurgie pour le traitement du cancer du sein
- âge d'au moins 18 ans et capable de lire et comprendre les informations sur l'étude en Français, Anglais, ou Allemand
- risque haut (>30%) de développer des douleurs chroniques après la chirurgie, identifié avec au moins 2 points d'un score de risque calculé par addition des facteurs suivants: douleur au site chirurgical avant la chirurgie (2 points), l’antécédent d’une dépression (1 point), l’âge de moins de 50 ans (1 point), et la crainte des douleurs fortes postopératoires (1 point)

- insuffisance cardiaque avec symptômes

Inclusion Criteria:

- patients scheduled for breast surgery for cancer, either breast-conserving
(tumorectomy/ quadrantectomy) or mastectomy, with or without immediate reconstruction,
and with or without axillary dissection.

- patients of 18 years or more scheduled for above mentioned type of surgery

- ability to speak and read French, English or German

- high (>30%) risk of clinically important persistent pain: identified with 2 or more
points of a risk score including the items: pre-existing pain at surgical site (2
points), history of depression (1 point), age < 50 years (1 point), and high expected
acute pain (>6/10, 1 point).

- Informed Consent as documented by signature.

Exclusion Criteria:

- Inability to understand the consent form and to follow the procedures of the study,
e.g. due to language problems, psychological disorders, dementia, etc. of the
participant

- Participation in another study with investigational drug within the 30 days preceding
and during the present study,

- Previous enrolment into the current study,

- Enrolment of the investigator, his/her family members, employees and other dependent
persons

- Pregnancy or lactation - Renal insufficiency (creatinine clearance < 60 ml/min)

- Allergy to pregabalin or the ingredients of the capsules

- Long-term preoperative therapy with gabapentinoids or high-dose opioids (more than 60
mg of morphine equivalents)

- Symptomatic cardiac insufficiency (peripheral oedema, NYHA class III - marked
limitation of physical activity)

- Suicidal ideation, identified by the question: "have you been bothered by thoughts
that you would be better off dead, or of killing yourself?"

- Planned fertility preservation immediately after surgery before a planned chemotherapy

- Known or suspected non-compliance, or substance-use disorder with impact on medication
adherence
