Morbus Crohn (MC) ist eine lang anhaltende, entzündliche Erkrankung von verschiedenen Darmabschnitten. Bislang ist für MC keine medizinische oder chirurgische Heilmethode bekannt. Die Behandlungsziele für MC umfassen derzeit die Linderung der Symptome, die Verringerung der Entzündung und die Verbesserung der Lebensqualität.
Upadacitinib (ABT-494) ist ein neuer Wirkstoff, der in dieser Studie eingesetzt wird. Er wurde für erwachsene Patienten mit entzündlichen Erkrankungen entwickelt, um bei Patienten, die an MC leiden, die Entzündung zu kontrollieren und die Symptome zu lindern.
Die Studie besteht aus 2 Teilen, die bis zu 24 Wochen dauern. Wenn die Patienten nach 12 bzw. 24 Wochen ein Ansprechen aufweisen, werden sie in die Studie M14-430 aufgenommen. Bei fehlendem Ansprechen scheiden sie aus.

Die Studie besteht aus 2 Teilen, die bis zu 24 Wochen dauern. Sie wird 300 Patienten an etwa 400 Prüfzentren weltweit umfassen.
Jeder Patient nimmt an einem Screeningverfahren teil, das zwischen 7 und 35 Tagen dauert. Daraufhin werden die Patienten nach dem Zufallsprinzip einem von 2 Behandlungszyklen zugewiesen und es erfolgt eine 12 wöchige Behandlungsphase mit 5 Visiten am Prüfzentrum. Die Behandlungszuweisung und Dosis, die ein Patient erhält, sind während der gesamten Studie gegenüber allen Beteiligten verblindet, auch wenn die Behandlung des Patienten wie unten beschrieben geändert wird. Die Patienten werden in Teil 1 nach dem Zufallsprinzip im Verhältnis 2:1 entweder Upadacitinib (200 Patienten) oder Placebo (100 Patienten) zugewiesen. Hat ein Patient die 12 wöchige Behandlung abgeschlossen und darauf angesprochen, wird er in die Studie M14-430 aufgenommen. Patienten ohne Ansprechen erhalten die Möglichkeit, in den 12 wöchigen Teil 2 mit 3 Visiten am Prüfzentrum einzutreten, in welchem sie Upadacitinib in hoher oder mittlerer Dosis erhalten (je nach zuvor erhaltener Dosis). Wenn diese Patienten nach den weiteren 12 Wochen ein Ansprechen aufweisen, werden sie in die Studie M14-430 aufgenommen. Patienten ohne Ansprechen scheiden aus und nehmen nach 30 Tagen eine Nachbeobachtungsvisite wahr.

Criteria for participation in trial

1. Männer oder Frauen, die zwischen 18 und 75 Jahre alt sind mit einer Morbus Crohn Diagnose mindestens 3 Monate vor der ersten Einnahme des Studienmedikamentes.
2. Bestätigte Diagnose von mittelschwerem bis schwerem aktivem MC, beurteilt durch die Stuhlfrequenz, der Intensität der Bauchschmerzen und der Auswertung einer einfachen Endoskopie für MC.
3. Nachgewiesene Unverträglichkeit gegenüber oder unzureichendes Ansprechen auf konventionelle Therapien (orale, lokal wirksame Steroide, intravenöse oder orale Kortikosteroide, Immunsuppressiva).
Exclusion criteria

1. Patienten mit aktuell diagnostizierter Colitis ulcerosa oder unbestimmter Colitis.
2. Patienten mit unregelmässiger Medikamenteneinnahme zur Behandlung des MC.
3. Patienten mit aktuell bekannten Komplikationen des MC (z. B. Strikturen, Abszessen, Kurzdarmsyndrom usw.).
Inclusion/Exclusion Criteria

Inclusion Criteria:
- Confirmed diagnosis of CD for at least 3 months prior to Baseline.
- Confirmed diagnosis of moderate to severe CD as assessed by stool frequency (SF),
abdominal pain (AP) score.
- Evidence of mucosal inflammation based on the Simplified Endoscopic Score for Crohn's
disease (SES-CD) on an endoscopy confirmed by a central reader.
- Demonstrated an inadequate response or intolerance to one or more conventional and/or
biologic therapies (Oral locally acting steroids, Intravenous or oral corticosteroids,
Immunosuppressants), in the opinion of the investigator.
Note: Participants who have had inadequate response or intolerance to conventional therapy
who have received prior biologic may be enrolled; however, participants must have
discontinued the biologic for reasons other than inadequate response or intolerance (e.g.,
change of insurance, well controlled disease).
- If female, participant must meet the contraception recommendations.
Exclusion Criteria:
- Participant with a current diagnosis of ulcerative colitis or indeterminate colitis.
- Participant not on stable doses of CD related antibiotics, oral aminosalicylates,
corticosteroids or methotrexate (MTX).
- Participant with the following ongoing known complications of CD: abscess (abdominal
or peri-anal), symptomatic bowel strictures, fulminant colitis, toxic megacolon, or
any other manifestation that might require surgery while enrolled in the study.
- Participant with ostomy or ileoanal pouch.
- Participant diagnosed with conditions that could interfere with drug absorption
including but not limited to short gut or short bowel syndrome.
- Screening laboratory and other analyses show abnormal results.
