Bei der Colitis ulcerosa (Cu) handelt es sich um eine chronisch-entzündliche Darmerkrankung (CED), die zu Entzündungen und Blutungen an der Rektum- und Colonschleimhaut (Dickdarm) führt. Derzeit bestehen unterschiedliche Behandlungsoptionen für Patienten mit Cu: u. a. entzündungshemmende Arzneimittel (Mesalazin oder Steroide), Immunsuppressiva (Azathioprin) und Biologika (Arzneimittel, die per Injektion gegeben werden und die Immunantwort verändern, z. B. Adalimumab, Infliximab, Vedolizumab), allerdings sind sie nicht immer wirksam. Risankizumab ist ein Studienmedikament, das zur Behandlung von Patienten mit entzündlichen Erkrankungen wie Cu entwickelt wird. In dieser Studie wird bei Studienteilnehmern mit mittelschwerer bis schwerer Cu, die auf eine vorherige Therapie mit Biologika nicht angesprochen haben, untersucht, wie gut Risankizumab wirkt und wie sicher es im Vergleich zu Placebo (kein Wirkstoff) ist.

In die Studie werden etwa 720 Teilnehmer an 400 Studienzentren weltweit aufgenommen.
In der verblindeten (d. h. niemand weiss, was der Studienteilnehmer erhält) Substudie 1 werden 240 Patienten nach dem Zufallsprinzip einem Behandlungsarm zugeteilt und erhalten über 12 Wochen entweder Placebo oder Risankizumab intravenös (i.v.). Studienteilnehmer, die nach 12 Wochen kein Ansprechen zeigen, können in eine zweite Behandlungsphase von 12 Wochen übergehen, in der alle Studienteilnehmer, auch jene, die in der ersten Behandlungsphase nur Placebo erhalten haben, Risankizumab entweder i.v. oder subkutan (s.c., d. h. Injektion unter die Haut) erhalten.
In Substudie 2 erhalten rund 480 Studienteilnehmer über 12 Wochen entweder Risankizumab oder Placebo i.v. Die verabreichte Dosis basiert auf den Ergebnissen aus Substudie 1. Alle Non Responder haben die Möglichkeit, Risankizumab (i.v. oder s.c.) für weitere 12 Wochen zu erhalten.
Wenn der verblindete Teil der Substudie 2 abgeschlossen ist, können weitere Patienten in die Studie aufgenommen werden und erhalten das Studienmedikament offen. Dadurch wird sichergestellt, dass eine angemessene Anzahl von Respondern an der Erhaltungsstudie M16 066 teilnimmt.
Alle Studienteilnehmer, die nach 12 oder 24 wöchiger Behandlung ein Ansprechen zeigen, können in die 52 wöchige Erhaltungsstudie M16 066 aufgenommen werden. Alle Patienten, die nach 24 wöchiger Behandlung kein Ansprechen zeigen, werden aus der Studie ausgeschlossen.

Criteria for participation in trial

1. Männer und Frauen ≥ 18 und ≤ 80 Jahre
2. Cu Diagnose, die mindestens 3 Monate vor Baseline bestätigt wurde
3. Aktive Cu
4. Nachgewiesene Unverträglichkeit gegenüber oder nachgewiesenes unzureichendes Ansprechen auf eine oder mehrere Biologikatherapien
Exclusion criteria

1. Aktuelle MC Diagnose, unklassifizierte chronisch-entzündliche Darmerkrankung (CED-U) oder Strahlenkolitis bzw. ischämische Kolitis in der Anamnese
2. Studienteilnehmer, die nicht zulässige Medikamente und Behandlungen erhalten
3. Auf das Rektum beschränktes Ausmass der entzündlichen Erkrankung gemäss Endoskopie beim Screening
4. Aktuell bekannte Komplikationen in Bezug auf Cu

Inclusion Criteria:
- Male or female aged >=18 to <= 80 years at the Baseline Visit. Where locally
permissible, subjects 16 to < 18 years of age who meet the definition of Tanner stage
5 for development at the Baseline Visit.
- Confirmed diagnosis of ulcerative colitis (UC) for at least 3 months prior to
Baseline.
- Active UC as assessed by Adapted Mayo Score and Endoscopic Subscore.
- Demonstrated intolerance or inadequate response to conventional therapy and
tofacitinib (not a biologic) and one or more biologic therapies.
- Females must be postmenopausal for more than 1 year or surgically sterile or
practicing specific forms of birth control.
Exclusion Criteria:
- Participant with a current diagnosis of Crohn's disease (CD), inflammatory bowel
disease-unclassified (IBD-U) or a history of radiation colitis or ischemic colitis.
- Participant receiving prohibited medications and treatment.
- Extent of inflammatory disease limited to the rectum as assessed by screening
endoscopy.
- Participant with currently known complications of UC (e.g., megacolon).
- No known active Coronavirus Disease - 2019 (COVID-19) infection.
