Brief description of trial (Data source: BASEC)
Bei dieser klinischen Studie, die in mehreren Zentren für Übergewichtschirurgie durchgeführt wird, wird die Wirkung des Medikamentes Xarelto (Rivaroxaban) 10 mg zur Verhinderung (Prophylaxe) von Thrombosen und Lungenembolien im Anschluss an die Übergewichtschirurgie untersucht. Patientinnen und Patienten erhalten im Anschluss an die Operation entweder während 7 oder während 28 Tagen das gerinnungshemmende Medikament Xarelto. Nach 28 Tagen erfolgt eine Kontrolle der Beinvenen mittels Ultraschall, um auch Thrombosen festzustellen, die nicht zu Beschwerden geführt haben. Bei einem Teil der Studienpatienten werden im Anschluss an die letzte Einnahme des Studienmedikaments über die Dauer von 24 Stunden zusätzliche Untersuchungen zur Konzentration und zur Wirkung von Xarelto durchgeführt.

Da Fettleibigkeit ein Risikofaktor für die Entwicklung von Thrombosen nach einer Übergewichtsoperation darstellt, wird je nach Zentrum eine medikamentöse Therapie zur Vorbeugung von Thrombosen und Lungenembolien während einer bis vier Wochen durchgeführt (z.B. mit Clexane® oder Fragmin®). Diese Medikamente müssen täglich als Spritze unter die Haut verabreicht werden.
Das in Tablettenform vorliegende Studienmedikament Xarelto® ist in der Schweiz zur Verhinderung von Thrombosen nach Hüft- oder Knieprothesenoperationen bereits zugelassen, jedoch noch nicht nach einer Übergewichtsoperation.
Diese Studie wird als nationale Studie an drei Studienzentren (Inselspital Bern, Klinik Beau-Site Bern, Kantonsspital Baden) durchgeführt und dauert insgesamt 28 Tage (plus ein Telefonanruf nach weiteren 7 Tagen).
Das Studienmedikament wird in derjenigen Dosierung (10 mg) verwendet, in der es zur Verhinderung von Thrombosen und Lungenembolien bei anderen Erkrankungen zugelassen ist.

- Herzinfarkt, Hirnschlag oder TIA während den letzten 6 Monaten vor Studienbeginn
- Unkontrollierbarer Bluthochdruck

- Patients with scheduled elective bariatric surgery or redo surgery after bariatric
interventions
- Written informed consent
Exclusion Criteria:
- DVT and/or PE in the patient history
- Myocardial infarction, transient ischemic attack or stroke within 6 months of study
