Brief description of trial (Data source: BASEC)
Die Schweiz hat die höchste Rate von neu aufgetretenen Melanomen in Europa (19,2 pro 100.000).1 Melanome haben die schlechteste Prognose aller Hautkrebserkrankungen.
Die derzeitige Behandlung ist abhängig von der histologischen Diagnose nach einer Biopsie und ist in erster Linie in Bezug auf die Tumordicke (Breslow Score), die sich in Teilung befindlichen Tumorzellen (Mitoserate), den Substanzdefekt der Haut (Ulzeration), das Auftreten einer Regression, und das Alter der Patienten bezogen.
Die Initialbehandlung erfolgt durch chirurgische Entfernung mit einem Sicherheitsabstand von makroskopisch gesunder Haut um den Tumor. Wenn die Tumordicke mehr als 1 mm oder mehr als 0,7 mm in Verbindung mit einer hohen Mitoserate bei jüngeren Patienten, Ulzerationen, Regression oder Clark Level IV / V besteht, dann legen die aktuellen Melanom-Richtlinien nahe, dass bei dem Patienten eine Sentinel-Lymphknoten-Biopsie (SLKB) durchgeführt wird, da dies höchstwahrscheinlich die erste Stelle ist, an der sich die Metastasen verbreiten. Das Merkelzellkarzinom ist ein sehr aggressiver, neuroendokriner Hauttumor mit einer Mortalität von etwa 33 % nach 3 Jahren.2 Auf Grund der häufigen lymphatischen Metastasen ist eine SLKB bei allen Patienten sehr zu empfehlen, um ihre Prognose besser beurteilen zu können.
Die Goldstandard Technik SLK zu identifizieren ist, das Radioisotop Technetium -99m um den Ort des Primärtumors unter die Haut zu injizieren. Der Patient wird anschliessend mittels Szintigraphie gescannt, um die Lage des SLK nach ca. 30 und 120 Minuten zu erfassen. Andere Teams haben versucht die transkutanen SLK mit Indocyaningrün und Nah-Infrarot-Fluoreszenz-Visualisierung zu identifizieren, sind aber zu dem Schluss gekommen, dass die ICG -Fluoreszenz -Technik bei Patienten mit einem hohen BMI oder bei einem Primärtumor mit Lymphabfluss in der axillären Lymphknotenregion nicht zuverlässig ist.

Diese Studie zielt darauf ab, ein medizinisches Gerät zu bewerten, welches im Vergleich zu früheren Studien eine verbesserte Technologie nutzt (Stereoskopische 3D-High-Definition sowohl für Fluoreszenz und sichtbare Licht- Bildgebung). Bisher hat es sich bewährt Lymphknoten intraoperativ zu identifizieren, aber wir versuchen nun zu ermitteln, ob diese Lymphknoten transkutan identifiziert werden können.

Inclusion Criteria:
- Malignant melanoma patients having one of the following characteristics:
- Breslow score = 1 mm
- Breslow score = 0.7 mm associated with ulceration
- Breslow score = 0.7 mm associated with regression
- Breslow score = 0.7 mm associated with Clark Level IV / V
- Breslow score = 0.7 mm associated with mitotic rate = 1/mm2 in young patients
- Merkel cell carcinoma
Exclusion Criteria:
- Age < 18 years
- Pregnancy and breastfeeding (pregnancy test to be performed for women of child-bearing
potential, defined as women who are not surgically sterilized/ hysterectomized, and/or
who are postmenopausal for less than 12 months)
- Known allergy to ICG or Iodine
- Previous chemotherapy, radiotherapy or surgery to the lymph nodes of interest
- Lack of capacity to provide informed consent
- Current enrolment in any other interventional study
