MEDPass-Studie: MEDPass versus herkömmliche Verabreichung von Trinknahrung: eine randomisiert kontrollierte Studie für den Vergleich von Energie- und Eiweissaufnahme.

Trinknahrungen werden bei einem erhöhten Risiko für eine Mangelernährung oft ergänzend zum normalen Essen eingesetzt, da sie reich an Energie und Eiweiss sind. Trinknahrungen sind in der Schweiz zugelassen und gesetzlich in der Lebensmittel- und Gebrauchsgegenständeverordnung verankert. Wir wollen mit dieser Studie untersuchen, ob ein neuer Verabreichungsmodus von Trinknahrung die Energie- und Eiweissaufnahme im Vergleich zur herkömmlichen Verabreichung von Trinknahrung verbessert. Bis anhin wurde die Trinknahrung im Spital, wo die Studie durchgeführt wird, immer im herkömmlichen Verabreichungsmodus serviert. Die MEDPass-Studie ist eine randomisierte, kontrollierte, unverblindete Studie. Es werden zwei Gruppen gebildet und zwei unterschiedliche Verabreichungsarten der Trinknahrung verglichen. „Randomisieren“ bedeutet, dass ausgelost wird, wer in welche Gruppe kommt. Es ist also Zufall, in welche Gruppe man eingeteilt wird. „Unverblindet“ heisst, dass es nicht geheim ist, in welcher Gruppe die Studienteilnehmerinnen und -teilnehmer eingeteilt sind. Die Studie dauert 30 Tage.

Im MEDPass-Modus wird die Trinknahrung viermal täglich fix in Portionen von 50 ml mit den Medikamenten serviert.
Bei der herkömmlichen Verabreichung wird die Trinknahrung als Zwischenmahlzeit in grösseren Portionen serviert. Die Portionengrösse kann abgestimmt auf die Patientinnen und Patienten jeweils 100-220 ml betragen. Das können ein bis drei Zwischenmahlzeiten sein.

Criteria for participation in trial (Data source: BASEC)

• Risiko für eine Mangelernährung ist vorhanden
• voraussichtlich dauert der Spitalaufenthalt noch mehr als 3 Tage
• Patientin, Patient ist über 18 Jahre alt

Exclusion criteria (Data source: BASEC)

• unstabile Stoffwechselsituation (zum Beispiel direkt nach einer Operation
oder nach einem Aufenthalt auf der Intensivstation)
• Vorhandensein einer Essstörung
• Patientin, Patient braucht eine künstliche Ernährung mit einer Sonde oder
mit einem Katheter direkt ins Blut

Inclusion Criteria:

- Nutritional risk screening (NRS-2002) total score =3 points according to routine
screening at admission within 72 hours

- Expected hospital LOS =3 days after screening (as estimated by the treating physician)

- Patient qualifies for ONS and approves prescription

- Age >18 years

- Willingness and ability to provide informed consent

Exclusion Criteria:

- Initially admitted to critical care unit

- Immediate post-operative phase (<7 days post-surgery)

- Dysphagia with the inability to swallow liquids

- Supplemental enteral and/or parenteral nutrition

- Admitted with, or scheduled for, total parenteral nutrition or tube feeding

- Mini Mental State examination < 16 points

- hospitalized due to anorexia nervosa

- hospitalized due to acute pancreatitis

- hospitalized due to acute liver failure

- cystic fibrosis

- patients after gastric bypass surgery

- patients with short bowel syndrome

- terminal condition (end of life situation)

- poor skills in German language (study language)
