Brief description of trial (Data source: BASEC)
Bei Patienten, bei denen die Herzrhythmusstörung Vorhofflimmern bekannt ist, besteht ein erhöhtes Schlaganfall-Risiko durch Gerinnselbildung insbesondere im linken Vorhofsohr, so dass hier der mechanische Verschluss des linken Vorhofohrs eine gute Alternative zur sonst dauerhaft erforderlichen Einnahme von stark wirksamen, blutverdünnenden Medikamenten zur Schlaganfall-Vorbeugung darstellt. Diese Behandlung wird standardmässig in der klinischen Praxis durchgeführt.
Die Gerinnselbildung auf dem Verschlussschirm ist eine seltene Komplikation. Um die Bildung von Blutgerinnseln auf dem Verschlussschirm zu verhindern, wird im Anschluss an diesen Eingriff kurzzeitige Blutverdünnung durchgeführt. Es ist unbekannt, ob Antikoagulatien (starke Blutverdünner) oder lediglich mildere Plättchenhemmer die ideale Nachbehandlung darstellen.
Das Ziel dieser Studie ist daher, die medikamentöse Behandlung mit sogenannten „Antikoagulantien“ (stark wirksamen Blutverdünnern mit Wirkung auf Blutgerinnungsfaktoren) mit einer weniger stark gerinnungshemmend wirkenden doppelten Blutplättchen-Hemmung (Aspirin plus Clopidogrel) zu vergleichen. Beide Medikamentenregimes werden für 8 Wochen nach dem Verschluss des linken Vorhofsohrs durchgeführt.

Medikamentöse Behandlung mit stark wirksamen Blutverdünnern
im Vergleich zu einer doppelten Blutplättchen Hemmung mit Aspirin und Clopidogrel bei Patienten nach Vorhofohrverschluss bei bekannten Herzrhythmusstörungen

Interventions

Drug: Clopidogrel -75 mg/day
Drug: Low dose aspirin -80 to 125 mg/day-
Drug: Rivaroxaban, dabigatran, apixaban, or edoxaban

- Patienten nach erfolgreichem Verschluss des Vorhofohrs
- Alter ≥ 18 Jahre
Exclusion criteria

-absolute Unverträglichkeit / Kontraindikationen auf Medikamente zur Blutverdünnung
- Hirnblutungen in der Vergangenheit
- schwere oder lebensbedrohliche Blutungen in den ersten 24 Stunden nach dem Vorhofsohrverschluss
Inclusion/Exclusion Criteria

Inclusion Criteria:
- Successful transcatheter LAAC with any approved device
- Age=18 years old
Exclusion Criteria:
- Absolute contraindications for anticoagulation therapy
- Prior intracranial hemorrhage
- Contraindications for TEE
- Severe pericardial effusion within the first 24 hrs following LAAC
- Major/life-threatening bleeding within the first 24 hrs following LAAC
- Female subjects of childbearing potential without using highly effective methods of
contraception
