Brief description of trial (Data source: BASEC)
Bei der schweren Covid-19 Erkrankung erfolgt bei allen Patienten eine Standardtherapie entsprechend der zu diesem Zeitpunkt gültigen Vorgaben des Instituts für Intensivmedizin des USZ. In unserem Forschungsvorhaben wollen wir herausfinden, ob ein zusätzlicher therapeutischer Plasmaaustausch eine potentiell schützende Wirkung auf die eingeschränkte Organfunktion hat und diese sogar verbessern kann.
Wenn der Patient teilnimmt, wird er zufällig in die Behandlungs-/oder die Kontrollgruppe eingeteilt. Das bedeutet, dass er in der Kontrollgruppe nach allen aktuell gültigen Standardvorgaben behandelt wird. In der Behandlungsgruppe erfolgt zusätzlich an drei Tagen ein sogenannter Plasmaaustausch
Diese Behandlung ähnelt einer Blutwäsche und dauert jeweils ca. 120-180 min, sie erfolgt an Tag 1; 3; und 5 auf der Intensivstation. An Tag 1, 3, 5 und 7 wird zusätzlich eine Kapillarmikroskopie mit einer Kamera unter der Zunge durchgeführt. Diese Messungen sind nicht-invasiv und absolut schmerzfrei.
Als Hauptendpunkt der Studie haben wir eine Veränderung von Gerinnungswerten definiert, die für die Durchblutung der kleinsten Blutgefässe in unseren Organen entscheidend sind (das sogenannte ADAMTS13/vWF:Ag Verhältnis).

Criteria for participation in trial

- SARS CoV2 Infektion
- schwerer Krankheitsverlauf (beatmet auf der Intensivstation)
- Zeichen der Entzündung (CRP > 100 mg/L)
- Zeichen der Gerinnungsaktivierung (D-Dimer > 10 mg/L)
Exclusion criteria

- Teilnahme an einer anderen Interventionsstudie
- Schwangerschaft
- extrakorporale Membranoxygenierung (ECMO)
Inclusion/Exclusion Criteria

Inclusion Criteria:
- Proven SARS-CoV2 infection
- Severe Covid-19 (indicated by respiratory failure requiring invasive mechanical
ventilation)
- Evidence of coagulopathy (indicated by D-dimer > 10 mg/L)
- Evidence of systemic inflammation (CRP > 100 mg/L , ferritin > 500 ng/mL)
Exclusion Criteria:
- • Participation in another study with investigational drug within the 30 days
preceding and during the present study,
- Previous enrolment into the current study.
- Extra corporeal membrane oxygenation (ECMO)
