Das Delir ist für Ärztinnen und Ärzte sowie Pflegefachpersonen eine grosse Herausforderung bezüglich Detektion, Therapie und damit zusammenhängend der Verhinderung von Folgeschäden. Über die Jahre wurden durch Studien zahlreiche Risikofaktoren nachgewiesen, aufgrund deren Vielfältigkeit die Wichtigkeit einer verlässlichen präventiven und therapeutischen Massnahme unterstrichen wird. Aus Studien mit dem Fokus auf die medikamentöse Delirtherapie ergeben sich zwar gewisse Möglichkeiten, jedoch nur unklare Richtlinien dafür, welches zugelassene Präparat besser bzw am besten für die Verkürzung und die Reduktion des Schweregrads des Delirs auf Intensivstationen geeignet ist. Daher möchten wir in unserer Studie zwei dieser Medikamente, Dexmedetomidin (Dexdor ®) und Propofol (Disoprivan ®), miteinander vergleichen. Zur besseren Beurteilung der neurologischen Situation wie auch zur Interpretation von kognitiven Störungen, welche nicht dem Delir entsprechen, wird während der Verabreichung des Studienmedikaments oder der Standardtherapie jeweils ein EEG (Hirnstromkurve) abgeleitet.

Überempfindlichkeit gegenüber Medikament, Vorerkrankung am Herzen, niedriger Blutdruck, Vorerkrankungen an den Hirngefässen, Eier- oder Sojaallergie, Alter unter 18 Jahren, schwere Grunderkrankung, Schwangerschaft, psychische Erkrankung, Epilepsie, regelmässiger Alkoholkonsum

Inclusion Criteria:
- Adult patients (age 18 years or older) in a state of delirium (hyperactive ore mixed
type) upon detection in the ICU (i.e., ICDSC >3).
Exclusion Criteria:
- Hypersensitivity to the active substance
- Advanced heart block (grade 2 or 3) unless implanted pacemaker
- Uncontrolled hypotension
- Severe cardiac dysfunction
- Bradycardia
- Egg allergy
- Soybean/soy allergy
- Age below 18 years
- Terminal state
- Pregnancy
- Active psychosis (of non organic origin = functional disturbances)
- Status epilepticus or postictal states following seizures on EEG
