Die Bedeutung der Darm-Hirn-Achse und der Effekt einer kohlenhydratarmen Diät bei Übergewicht und Depression

Brief description of trial (Data source: BASEC)

In diesem Projekt werden wir die Auswirkungen einer kohlenhydratreduzierten Kost untersuchen. Depressive Störungen und Übergewicht kommen gehäuft zusammen vor und man geht davon aus, dass etwa ein Drittel aller Individuen mit Depressionen zum „metabolen“, d.h. übergewichtigen Subtyp gehört. Wir machen dieses Projekt, um herauszufinden, ob Übergewichtige und depressive Patienten von dieser Diätanpassung profitieren können. Die Studie besteht aus einem Screening und 6 Studientagen. Über eine Anwendungssoftware (= „App“, Application software) werden die Teilnehmer geschult und stellen ihre Ernährung langsam um, bis nach ca. 2 Monaten eine Einnahme von max. 130g Kohlenhydraten pro Tag erreicht wird. Vor und nach dieser Ernährungsumstellung, die insgesamt 6 Monate eingehalten werden soll, werden zahlreiche Untersuchungen vorgenommen. Dazu gehören Blutabnahmen (Vitamine und Spurenelement, Blutfette, Blutzucker, usw.), Bestimmung der Körperzusammensetzung (Fettanteil, Muskelanteil), Bestimmung des Leberfettanteils und eine funktionelle Untersuchung des Gehirns mittels MRI (Magnenresonanztomographie), sowie Stuhlproben zur Bestimmung der Darmbakterienzusammensetzung, Fragebögen zu Magendarmbeschwerden und zur psychischen Verfassung.

Das Forschungsprojekt untersucht die Wirkung einer kohlenhydratreduzierten Kost („Low Carb“) auf den Stoffwechsel und die Psyche.

Criteria for participation in trial (Data source: BASEC)

Dieses Projekt wird in der Schweiz (Basel) und in Deutschland (Lübeck) durchgeführt. In der Schweiz werden folgende Teilnehmer eingeschlossen:

Kontrollgruppe:
Normalgewichtige Personen (d.h. BMI = Body Mass Index 19.0-24.9kg/m2), welche zwischen 18 und 55 Jahre alt und körperlich und psychisch gesund sind (Kontrollgruppe) oder eine Depression haben (Depressive, Schlanke).

Übergewichtige:
Übergewichtige Personen (d.h. BMI = Body Mass Index >30kg/m2), welche zwischen 18 und 55 Jahre alt sind und einen erhöhten Blutzuckerspiegel haben (Vorstufe zu Diabetes, oder Diabetes) ohne oder mit Depression.

Exclusion criteria (Data source: BASEC)

Nicht mitmachen können Personen, die:
- bereits eine kohlenhydratreduzierte Kost zu sich nehmen oder sonst eine spezielle Diätform verwenden
- an einer psychischen Erkrankung leiden
- regelmässig Magensäureblocker zu sich nehmen
- regelmässig Probiotika oder Präbiotika zu sich nehmen (Bakterienpräparate für den Darm)

Inclusion/Exclusion Criteria (Data source: WHO)

Gender: All
Maximum age: 55 Years
Minimum age: 18 Years

Inclusion Criteria:

Group A: HEALTHY LEAN CONTROLS

- Healthy normal weight subjects with a body-mass index of 19.0-24.9kg/m2, HbA1C <5.7%
and fasting glucose <5.6 mmol/l

- Normal eating habits

- Stable body weight for at least three months

- Informed Consent as documented by signature

Group B: PRE-DIABETIC or DIABETIC OBESE

- Pre-diabetic/Diabetic obese with a HbA1C >5.7% and/or fasting glucose >5.6 mmol/l) and
body-mass index > 30kg/m2, otherwise healthy

- Normal eating habits

- Stable body weight for at least three months

- Informed Consent as documented by signature

Exclusion Criteria:

Group A: HEALTHY LEAN CONTROLS

- Pre-existing low carb diet (less than 45% of daily energy intake by carbohydrates)

- Pre-existing diet (vegetarian, vegan, gluten-free etc.)

- Psychiatric illness

- Alcohol abuse, (smoking allowed)

- Regular intake of medications, (oral contraceptives allowed)

- Intake of antibiotics within the last 3 months before inclusion

- Regular intake of pro- or prebiotics

- Chronic diseases of the gastrointestinal tract, history of gastrointestinal surgery
with major changes to the gastrointestinal tract (e.g. bariatric surgery)

- Clinically relevant acute or chronic inflammatory disease

- Pregnancy

- Participation in another study with investigational drug within the 30 days preceding
and during the present study.

Group B: PRE-DIABETIC or DIABETIC OBESE

- Pre-existing low carb diet (less than 45% of daily energy intake by carbohydrates)

- Pre-existing diet (vegetarian, vegan, gluten-free etc.)

- Psychiatric illness

- Alcohol abuse, (smoking allowed)

- Regular intake of medications (except: oral contraceptives, metformin, SGLT-2,
statins, and antihypertensive, which are allowed)

- Intake of antibiotics within the last 3 months before inclusion

- Regular intake of pro- or prebiotics

- Chronic diseases of the gastrointestinal tract, history of gastrointestinal surgery
with major changes to the gastrointestinal tract (e.g. bariatric surgery)

- Clinically relevant acute or chronic inflammatory disease

- Pregnancy

- Participation in another study with investigational drug within the 30 days preceding
and during the present study.
