Studie zur Beurteilung der Sicherheit und Wirksamkeit von BLU-263 bei Patienten mit indolenter und systemischer Mastozytose

Brief description of trial (Data source: BASEC)

Diese Studie wurde entwickelt, um die empfohlene Dosis (RD) von BLU-263 zu bestimmen sowie die Sicherheit und Wirksamkeit zu beurteilen, bei Patienten mit indolenter systemischer Mastozytose (ISM) oder Patienten mit einem Mastzellaktivierungssyndromm (MCAS). Die Studie besteht aus vier Teilen und zwei optionalen offenen Pharmakokinetik (PK)-Gruppen: • Teil 1 der Studie enthält 3 Dosisgruppen und eine Placebogruppe zur Bestimmung der RD von BLU 263 bei Patienten mit ISM. • In Teil 2 der Studie wird beurteilt, ob die Behandlung mit BLU-263 zusätzlich zur besten unterstützenden Behandlung (BSC) die Ergebnisse bei Patienten mit ISM im Vergleich zu Placebo+ BSC verbessert. Teil 2 erlaubt bis zu 99 Patienten mit niedrigeren TSS-Werten. • Teil 3 der Studie ist eine offene Erweiterung zur weiteren Charakterisierung der langfristigen Sicherheit und Wirksamkeit von BLU 263. Die Patienten gehen von Teil 1 und Teil 2 über in Teil 3 der Studie. • Teil M wird die Behandlung von BLU-263 bei Patienten mit mMCAS in einem offenen Design untersuchen. • Eine optionale PK-Gruppe von bis zu 20 Patienten kann vor und/oder parallel zu Teil 1 aufgenommen werden. Eine zusätzliche optionale PK-Gruppe von bis zu 20 Patienten kann auch vor Teil 2 aufgenommen werden. Beide PK-Gruppen sollen die Pharmakokinetik (PK) von BLU-263 bei Patienten mit ISM besser charakterisieren. Patienten in der/den optionalen PK-Gruppe(n) können einen größeren Bereich von TSS-Werten aufweisen als die randomisierten Teile der Studie oder haben ausgewählte Komorbiditäten oder Begleitmedikationen, die die BLU-263 PK beeinflussen können.

Indolente und schwelende systemische Mastozytose. Bei der systemischen Mastozytose (SM) handelt es sich um eine klonale neoplastische Mastzell-(MZ-)Erkrankung, welche durch eine erhöhte MZ-Last mit fokalen und/oder diffusen Infiltraten neoplastischer MZ in Haut, Knochenmark (KM), Milz, Leber Gastrointestinal-(GI-)Trakt und anderen Organen und eine erhöhte Freisetzung von MZ-Mediatoren charakterisiert ist.Das Knochenmark ist bei allen Patienten beteiligt. Die Weltgesundheitsorganisation (WHO) hat Kriterien für die Diagnose und Klassifizierung von SM entwickelt. In der zuletzt von der WHO vorgelegten Aktualisierung wird SM eingeteilt in indolente SM (ISM), schwelende SM (SSM), systemische Mastozytose mit assoziierter klonaler, nicht der Mastzellreihe zuzuordnender, hämatologischer Erkrankung (SM AHN), aggressive SM (ASM) und MZ-Leukämie (MCL).

Diese randomisierte, doppelblinde, placebokontrollierte Studie der Phase 2/3 von BLU-263 in indolenter systemischer Mastozytose wird durchgeführt, um die empfohlene Dosis (RD) von BLU-263 zu bestimmen (Teil 1); die Sicherheit und Wirksamkeit im Gegensatz zu Placebo in Verbindung mit der besten unterstützenden Behandlung (BSC) zu beurteilen (Teil 2); und die Sicherheit und Wirksamkeit der Langzeitbehandlung mit BLU-263 weiter zu charakterisieren (Teil 3).
Teil M der Studie wird die Behandlung von BLU-263 bei Patienten mit einem Mastzellaktivierungssyndrommm (MCAS) in einem offenen Design untersuchen. Und zwei optionale PK-Gruppen sollen die Pharmakokinetik (PK) von BLU-263 bei Patienten mit ISM besser charakterisieren.

Criteria for participation in trial (Data source: BASEC)

1. Der Patient muss zum Zeitpunkt der Unterzeichnung der Einwilligungserklärung einschließlich ≥18 Jahre alt sein. (Patienten, die ≥16 und <18 Jahre alt sind, haben ebenfalls die Möglichkeit an der Studie teilzunehmen)
2. Der Patient muss einen Leistungsstatus der Eastern Cooperative Oncology Group (ECOG PS) von 0 bis 2 haben.
3. In Teil 1 muss der Patient während des Ansprechzeitraums einen durchschnittlichen TSS von mindestens 14 Tagen von ≥ 28 und zusätzlich ≥ 1 Symptom in Haut- oder GI-Domänen der ISM-SAF zu Studienbeginn haben. In Teil 2 können Patienten mit einem durchschnittlichen TSS von mindestens 14 Tagen ≥ 28 und < 28 während des Ansprechzeitraums aufgenommen werden. In Teil M müssen die Patienten mMCAS haben, bestätigt durch die zentrale Pathologieprüfung der BM-Biopsie. Eine Archivbiopsie kann verwendet werden, wenn sie innerhalb der letzten 12 Monate abgeschlossen wurde. In PK-Teil Patienten mit einem 14-tägigen durchschnittlichen TSS von < 28 können eingeschlossen werden, wenn sie einen Single-Domain-Score von mindestens 7 haben.

Exclusion criteria (Data source: BASEC)

1. Bei dem Patienten wurde eine der folgenden WHO-SM-Unterklassifizierungen diagnostiziert:
a) Nur kutane Mastozytose (dh ohne Dokumentation einer systemischen Beteiligung)
b) Schwelende systemische Mastozystose (SSM)
c) Systemische Mastozytose mit einem assoziierten hämatologischen Neoplasma der Nicht-MC-Linie (SM-AHN)
d) Aggressive systemische Mastozytose (ASM)
e) Mastzellleukämie (MCL).
f) MC-Sarkom
2. Bei dem Patienten wurde eine andere myeloproliferative Erkrankung diagnostiziert (z. B. myelodysplastisches Syndrom, myeloproliferative Neoplasie).
3. Patient hat einen der folgenden auf SM zurückzuführenden C-Befunde der Organschädigung:
a) Zytopenie:
i. Absolute Neutrophilenzahl < 1,5 × 109/l
ii. Hämoglobin < 10 g/dl
iii. Thrombozytenzahl < 100.000/µl
b) Hepatomegalie mit Aszites und eingeschränkter Leberfunktion
c) Tastbare Splenomegalie mit Hypersplenismus
d) Malabsorption mit Hypoalbuminämie und signifikantem Gewichtsverlust.
e) Skelettläsionen: große osteolytische Läsionen mit pathologischen Frakturen.
f) Lebensbedrohlicher Organschaden in anderen Organsystemen, der durch MC-Infiltration in Gewebe verursacht wird
4. QT-Intervall > 480 ms.
5. Der Patient hat ein QT-Intervall, das mit der Fridericia-Formel (QTcF) > 480 ms korrigiert wurde.

Key Inclusion Criteria:

All Patients

-1. Patient must have an Eastern Cooperative Oncology Group Performance Status (ECOG PS) of
0 to 2.

Part 1 and Part 2

- 2. Patient must have moderate-to-severe symptoms based on minimum mean total symptom
score (TSS) of the ISM Symptom Assessment Form (ISM-SAF) over the 14-day eligibility
screening period.

- 3. Patient has confirmed diagnosis of ISM, confirmed by Central Pathology Review of BM
biopsy and central review of B- and C-findings by WHO diagnostic criteria. Archival
biopsy may be used if completed within the past 12 months.

- 4. Patient must have failed to achieve adequate symptom control for 1 or more Baseline
symptoms, as determined by the Investigator, with at least 2 of the following
symptomatic therapies administered: H1 blockers, H2 blockers, proton-pump inhibitors,
leukotriene inhibitors, cromolyn sodium, corticosteroids, or omalizumab.

- 5. Patients must have BSC for ISM symptom management stabilized for at least 14 days
prior to starting screening procedures.

- 6. For patients receiving corticosteroids, the dose must be = 20 mg/d prednisone or
equivalent, and the dose must be stable for = 14 days.

Part M

- 7. Patients must have mMCAS, confirmed by Central Pathology Review of BM biopsy. An
archival biopsy may be used if completed within the past 12 months.

- 8. Patients must have tryptase < 20 ng/mL.

- 9. Patients must have KIT D816V in peripheral blood (PB) or BM and/or CD25+ Mast cells
in BM.

- 10. Patients must have symptoms consistent with mast cell activation (despite BSC) in
at least two organ systems characterized by cutaneous flushing, tachycardia, syncope,
hypotension, diarrhea, nausea, vomiting and gastro-intestinal cramping) and serum
blood tryptase (sBT) levels above 8 ng/mL OR Severe (Ring and Messmer grading = II,
recurrent anaphylaxis, including but not limited to hymenoptera venom, drug or food,
regardless of sBT levels.

PK Groups

- 11. See inclusion criteria for All patients and Part 1/Part 2

- 12. Accrual may be limited to patients who have specific disease manifestations (ie,
GI involvement) or are taking acid-reducing agents to better explore the impact of
these features on PK.

Key Exclusion Criteria:

- 1. Patient has been diagnosed with any of the following WHO systemic mastocytosis (SM)
sub-classifications: cutaneous mastocytosis only, smoldering SM, SM with associated
hematologic neoplasm, aggressive SM, mast cell leukemia, or mast cell sarcoma.

- 2. Patient has been diagnosed with another myeloproliferative disorder.

- 3. Patient has organ damage C-findings attributable to SM.

- 4. Patient has clinically significant, uncontrolled, cardiovascular disease

- 5. Patient has a QT interval corrected using Fridericia's formula (QTcF) > 480 msec.

- 6. Patient has previously received treatment with any targeted KIT inhibitors.

- 7. Patient has a history of a primary malignancy that has been diagnosed or required
therapy within 3 years. The following prior malignancies are not exclusionary:
completely resected basal cell and squamous cell skin cancer, curatively treated
localized prostate cancer, and completely resected carcinoma in situ of any site.

- 8. Time since any cytoreductive therapy including mastinib and midostaurin should be
at least 5 half-lives or 14 days (whichever is longer), and for cladribine, interferon
alpha, pegylated interferon, or antibody therapy < 28 days or 5 half-lives of the drug
(whichever is longer), before beginning the screening period.

- 9.Patient has received radiotherapy or psoralen and ultraviolet A (PUVA) therapy < 14
days before beginning the screening period.
