Brief description of trial (Data source: BASEC)
Zweck dieser Studie ist es, die guten wie schlechten Wirkungen der Kombination aus Idasanutlin und dem Chemotherapeutikum Cytarabin mit denen von Placebo (einer inaktiven Substanz, die wie Idasanutlin aussieht) plus Cytarabin bei Patienten mit rezidivierender oder therapieresistenter akuter myeloischer Leukämie (AML) zu vergleichen, um festzustellen, welche Therapie besser geeignet ist. Die Patienten werden im Verhältnis 2:1 randomisiert; das bedeutet, sie werden zufällig (wie bei einem Münzwurf) einer Gruppe zugeteilt. Zwei Drittel der Patienten erhalten Idasanutlin plus Cytarabin und ein Drittel erhält Placebo plus Cytarabin. Weder die Patienten noch die Prüfärzte wissen, in welcher Behandlungsgruppe der jeweilige Patient ist.
Der Zeitraum, in dem die Patienten behandelt werden, ist in sogenannte «Zyklen» unterteilt. Ein Zyklus umfasst dabei 28 Tage: Die Patienten erhalten fünf Tage lang Idasanutlin und Cytarabin oder Placebo und Cytarabin; an den darauffolgenden 23 Tagen erhalten die Patienten keine Studienmedikamente. Diese 23 Tage werden auch als «Ruhetage» bezeichnet. Der erste Zyklus ist der sogenannte «Induktionszyklus». In diesem Zyklus erhalten die Patienten 600 mg Idasanutlin oder Placebo pro Tag; dies entspricht zwei Tabletten. Die erste Tablette Idasanutlin oder Placebo wird am Morgen eingenommen, und die zweite Tablette am Abend – ungefähr 10 bis 14 Stunden nach der morgendlichen Tablette. Cytarabin, das Chemotherapeutikum, wird nach Einnahme der ersten Tablette Idasanutlin bzw. Placebo am Morgen als intravenöse (i.v.) Infusion (in eine Vene) verabreicht. Die Infusion wird ca. eine bis drei Stunden dauern. Patienten, die auf die Behandlung ansprechen und im Anschluss keine Transplantation erhalten, können eventuell bis zu zwei weitere Zyklen Cytarabin plus Idasanutlin oder Placebo als Konsolidierungstherapie erhalten. In diesem Fall wird die Dosis Idasanutlin bzw. Placebo halbiert und es wird lediglich die morgendliche Tablette eingenommen.

Idasanutlin oder Placebo wird über einen Zeitraum von fünf Tagen in einer Dosis von 300 mg zweimal täglich oral eingenommen (Tag 1 bis 5); daran schliessen sich 23 Ruhetage an (bei Zyklus 1: Tag 6 bis 28). Patienten, die auf die Behandlung ansprechen, können im Anschluss eventuell bis zu zwei weitere Zyklen als Konsolidierungstherapie erhalten (Zyklus 2 und Zyklus 3).
Cytarabin wird über einen Zeitraum von fünf Tagen (Tag 1 bis 5) als einmal tägliche intravenöse Infusion von jeweils 1- bis 3-stündiger Dauer verabreicht; daran schliessen sich 23 Ruhetage an (bei Zyklus 1: Tag 6 bis 28). Patienten, die auf die Behandlung ansprechen, können im Anschluss eventuell bis zu zwei weitere Zyklen als Konsolidierungstherapie erhalten.
Während der Behandlungsphase (Zyklus 1 bis Zyklus 3) werden zu bestimmten Zeitpunkten verschiedene medizinische Untersuchungen, beispielsweise Blutabnahmen und Knochenmarkentnahmen, durchgeführt.

Criteria for participation in trial

•Alter ≥ 18 Jahre
•Dokumentierte/bestätigte erste oder zweite therapieresistente/rezidivierende AML gemäss Klassifizierung der Weltgesundheitsorganisation (WHO), ausgenommen akute Promyelozyten-Leukämie
•Nicht mehr als zwei vorherige Induktionsregimes (exkl. vorherige HSCT); bei einem davon muss Cytarabin zusammen mit einem Anthracyclin (oder Anthracendion) verabreicht worden sein
Exclusion criteria

•Patienten mit einer CR1-Dauer von > 1 Jahr UND Alter < 60 Jahre
•Patienten, die zuvor bereits mehr als zwei Induktionsregimes mit Cytarabin zur AML-Behandlung erhalten haben
•Schwangere oder stillende Patientinnen

Inclusion Criteria:
- Documented/confirmed first/second refractory/relapsed AML using World Health
Organization classification, except acute promyelocytic leukemia
- No more than 2 prior induction regimens (excluding prior HSCT) in their first line
treatment and one must have included cytarabine with an anthracycline (or
anthracenedione)
- Eastern Cooperative Oncology Group performance status of 0 to 2
- Adequate hepatic and renal function
- White blood cell (WBC) count at randomization less than or equal to (=) 50000 cells
per cubic millimeter (/mm^3)
Exclusion Criteria:
- First relapsed participants aged less than (<) 60 years with first CR duration greater
than (>) 1 year
- Participants with prior documented antecedent hematological disorder (AHD)
- AML secondary to any prior chemotherapy unrelated to leukemia
- Participants who are either refractory to or relapsed within 90 days of receiving a
regimen containing a cumulative dose of greater than or equal to (>/=) 18 g/m^2 of
cytarabine
- Participants who have received allogeneic HSCT within 90 days prior to randomization
- Participants who have received immunosuppressive therapy for graft versus host disease
or for engraftment syndrome after autologous stem cell transplantation within 2 weeks
prior to randomization
- Prior treatment with an Murine Double Minute 2 (MDM2) antagonist
- Participants receiving any other investigational or commercial agents or therapies
administered with the intention to treat their malignancy within 30 days from first
receipt of study drug
- Participants with a history of other malignancy within 5 years prior to screening
except for malignancy that has been in remission without treatment for at least 2
years prior to randomization
- Participants who have any severe and/or uncontrolled medical conditions or other
conditions that could affect their participation in the study
- Participants with extramedullary AML with no evidence of systemic involvement
- Pregnant or breastfeeding participants
