Rivaroxaban (Handelsname Xarelto®) ist ein Arzneistoff zur Hemmung der Blutgerinnung. Die Substanz ist in der Schweiz zur Vorbeugung und Behandlung von Blutgefässverschlüssen zugelassen, z.B. nach Einsatz von Gelenkprothesen. Aufgrund der Verstoffwechslung in der Leber und die Beteiligung von Transportproteinen kann es bei gleichzeitiger Anwendung von anderen Medikamenten zu relevanten Wechselwirkungen kommen.
Manche Substanzen, wie das pflanzliche Mittel Johanniskraut, können die Verstoffwechslung und den Transport anderer Substanzen beschleunigen. Es ist daher möglich, dass bei gleichzeitiger Einnahme von solchen Substanzen, welche Induktoren genannt werden, andere Arzneistoffe nicht ausreichend wirken können. Die Interaktionen zwischen Rivaroxaban und Induktoren wie Johanniskraut sind jedoch bis zum jetztigen Zeitpunkt nur wenig untersucht. Diese Aspekte können weitere Kenntnisse zu der Pharmakologie der Medikamente liefern und zu ihrer sicheren Anwendung beitragen.

Wir wollen mit dieser Studie untersuchen, ob die Konzentration bzw. die Wirkung des Blutverdünners Rivaroxaban (Xarelto®) durch die Kombination mit Johanniskraut stark verändert wird. Diese Erkenntnisse können in der Zukunft zu bestimmten Empfehlungen zur Dosisanpassung beitragen.

Die gesamte Studie umfasst 2 Sitzungen, eine ohne und eine mit Induktion. Jede Sitzung beginnt mit der Erfassung der Verstoffwechslung (Phänotypisierung); dafür bekommen die Probanden einmalig gleichzeitig niedrige Dosierungen von Fexofenadin (Mittel gegen allergische Reaktionen) durch den Mund und Midazolam (Dormicum®, Schlafmittel) durch den Mund. Eine Blutentnahme erfolgt vor der Gabe und 0.5, 2, 3 und 6 Stunden nach der Gabe. Nach 5 Tagen erhalten die Probanden einmalig am Morgen eine Tablette Xarelto® 20mg. Blutproben werden vor der Einnahme, 0.5, 1, 2, 3, 4, 6, 8, 12, 24, 36 und 48 Stunden nach Einnahme entnommen (eine Übernachtung im Spital ist nicht nötig, nach der Blutentnahme 12 Stunden nach Einnahme können die Probanden nach Hause gehen und am nächsten Morgen wieder kommen).
Die gleichen Abläufe werden wiederholt nach Einnahme von Johanniskraut (Jarsin®) 2 mal täglich 450mg durch den Mund über 2 Wochen.

Gesunde Frauen und Männer zwischen 18 – 45 Jahren, welche weder stark unter- noch übergewichtig sind.

Schwangerschaft, Einnahme von anderen Medikamenten, bekannte Lichtempfindlichkeit, Teilnahme an einer anderen Studie in den letzten 30 Tagen.
Inclusion/Exclusion Criteria

Inclusion Criteria:
- Men or women, age between 18 and 45 years (inclusive) at screening
- BMI between 18 and 28 kg/m2 (inclusive) at screening
- No clinically significant findings on the physical examination at screening
- Hematology and clinical chemistry results not deviating from the normal range to a
clinically relevant extent at screening
- Ability to communicate well with the investigator and to understand and comply with
the requirements of the study
- Women of child-bearing age: willingness of using a double barrier contraception method
during the study, i.e. a hormonal method (oral contraceptive, intrauterine device) in
combination with a mechanical barrier (e.g. condom, diaphragm)
- Signed informed consent
Exclusion Criteria:
- Known allergic reaction to any excipient of the drug formulations
- Known photosensitivity
- Smoking
- History or clinical evidence of alcoholism or drug abuse within the 3-year period
prior to screening
- Loss of = 250 ml of blood within 3 months prior to screening, including blood donation
- Treatment with an investigational drug within 30 days prior to screening
- Previous treatment with any prescribed or over-the-counter medications (including
herbal medicines such as St. John's wort) within 2 weeks prior to screening
- Pregnant (positive results from urine drug screen at screening) or lactating women
- History or clinical evidence of any disease (e.g. gastrointestinal tract disease)
and/or existence of any surgical or medical condition, which might interfere with the
absorption, distribution, metabolism or excretion of the study drugs, or which might
increase the risk for toxicity
- Legal incapacity or limited legal capacity at screening
- Any circumstances or conditions, which, in the opinion of the investigator, may affect
full participation in the study or compliance with the protocol
