Effekt des SGLT2 Inhibitors Empagliflozin auf Patienten mit euvolämischer und hypervolämischer Hyponatriämie - die EMPOWER Studie

Aktuell sind die Behandlungsmöglichkeiten der Hyponaträmie nur begrenzt wirksam und langwierig. Deswegen schlagen wir eine neue mögliche Therapie vor. Empagliflozin (Jardiance)® ist zur Behandlung von Patienten mit Diabetes mellitus (Zuckerkrankheit) zugelassen. Es kontrolliert den Blutzucker durch vermehrte Ausscheidung von Zucker im Urin. Dadurch wird ebenfalls vermehrt freies Wasser ausgeschieden, was eine mögliche Behandlung für die Hyponatriämie – einem zu tiefen Natriumspiegel im Blut durch Wasserüberschuss - darstellen könnte. Wir haben eine erste kurze Studie mit 87 Patienten durchgeführt, welche vermuten lässt, dass diese Behandlung erfolgreich ist. Allerdings braucht es nun die EMPOWER-Studie, um diese Ergebnisse in einer grösseren Patientengruppe sowie mit längerer Therapiedauer zu bestätigen. Die Hälfte der Patienten erhält eine Behandlung mit dem echten Medikament (Empagliflozin (Jardiance)® 25mg), die andere Hälfte erhält das Placebo. Das Placebo-Medikament enthält keinen Wirkstoff und ist somit ein „Scheinmedikament“. Nach dem Zufallsprinzip wird entschieden welche Behandlung welchem Patienten zugeordnet ist. Die Behandlungsdauer beträgt 30 Tage. Ein chronisch zu tiefer Natriumspiegel kann zu einer Beeinträchtigung des Gedächtnisses, der Muskelkraft, des Gangbildes sowie der Herzkreislauf-Leistung führen. Deswegen werden spezifischen Tests vor, während und nach der Therapie durchgeführt, die folgende Parameter beurteilen: Lebensqualität,Funktionen des Gehirns, Griffstärke und Gangstabilität. Das Haupt-Ziel dieser Studie ist zu untersuchen, ob durch eine 30-tägige Behandlung mit Empagliflozin (Jardiance)® 25mg eine bessere Korrektur des Natriumspiegels im Blut im Vergleich zu Placebo (Scheinmedikament) erreicht werden kann. Weitere Ziele sind zu untersuchen, ob die Korrektur des Natriumspiegels zu einer Verbesserung der oben genannten Beschwerden führt.

Criteri di esclusione (Fonte di dati: BASEC)

Nicht teilnehmen können Personen, deren Natriumspiegel notfallmässig korrigiert werden muss, deren Nebenerkrankungen oder aktuellen Gesundheitslage eine Kontraindikation zur Empagliflozin darstellt oder welche bereits mit einem Präparat dieser Medikametenklasse behandelt werden.

Inclusion Criteria:

- chronic eu- OR hypervolemic non hyperosmolar (<300 mOsm/kg) hyponatremia (heparin plasma
sodium <135 mmol/L on day of inclusion)

Exclusion Criteria:

- known hypersensitivity or allergy to class of drugs or the investigational product,

- severe symptomatic hyponatremia in need of treatment with 3% NaCl-solution or in need
of intensive/intermediate care treatment at time of inclusion

- clinical hypovolemia

- Severe reduction of eGFR <20 mL/min/1,73 m2 (KDIGO G4 and G5) or end stage renal
disease

- Chronic liver insufficiency with Child Pugh Score =10 or decompensated liver cirrhosis
(jaundice, hepatorenal syndrome, encephalopathy, bleeding, ?)

- Hepatic impairment defined as aspartate transaminase (AST) or alanine transaminase
(ALT) >3x the upper limit of normal (ULN); or total bilirubin >2x ULN at time of
enrolment

- uncontrolled hypothyroidism

- uncontrolled adrenal insufficiency

- systolic blood pressure <90mmHg

- contraindication for lowering blood pressure

- diabetes mellitus type 1 or pancreatic diabetes mellitus

- treatment with SGLT2 inhibitors, lithium chloride, vaptans, demeclocycline or urea on
inclusion day

- severe immunosuppression (leucocytes <2 G/l)

- peripheral arterial disease stage III-IV of the Fontaine Classification

- fasting or other reasons preventing medication intake

- previous enrolment into the current study

- participation in another intervention study

- pregnancy, breastfeeding, intention to become pregnant during the course of the study
or lack of safe contraception.
