Descrizione riassuntiva della sperimentazione (Fonte di dati: BASEC)
Internationale Studie mit Selonsertib (SEL) - noch von keiner Zulassungsbehörde zugelassen - zur Behandlung der fortgeschrittenen Leberfibrose infolge einer nicht-alkoholischen Steatohepatitis (NASH). 800 Teilnehmende (12 in der Schweiz) sind vorgesehen. Die Studie wird weltweit an etwa 460 Zentren durchgeführt. Randomisierte, doppelblinde, placebokontrollierte Studie.
Placebo ist eine Tablette, die keinen Wirkstoff enthält, aber wie Selonsertib aussieht. Doppelblind bedeutet, dass weder der Patient noch der Prüfarzt wissen werden, welches Prüfmedikament der Patient nimmt.
Das Ziel dieser Studie ist es herauszufinden, ob Selonsertib sicher und gut verträglich ist und ob es Fibrose und damit verbundene Komplikationen bei
Patienten mit Zirrhose und Fibrose infolge einer NASH verringern kann.
Die Teilnahme an dieser Studie wird etwa 240 Wochen dauern, Voruntersuchungstermin, Nachkontroll-Visite nach Behandlungsende (vier Wochen nach der letzten Dosis) und Telefongespräch als Nachkontrolle (zwölf Wochen nach dem Besuchstermin in Woche 240) nicht eingeschlossen. In dieser Zeit ist es erforderlich, das Spital mindestens 32 Mal aufzusuchen (Verfahren und Untersuchungen: EKG, Labor, Leberbiopsie, Magnetresonaz)
Wenn es zu einer erheblichen Verschlechterung der Leberfunktion kommt oder eine Leberbiopsie eine Zirrhose feststellt vor Woche 240 des Randomisierungs- abschnitts, können Patienten in einen offenen (OL) Abschnitt der Studie wechseln (Rollover). In diesem Abschnitt werden Patienten mit 18 mg SEL für 240 Wochen behandelt.
Häufigsten Nebenwirkungen (>10%): Kopfschmerzen, Durchfall, Übelkeit, Sinusitis (Nasennebenhöhlenentzündung), Nasopharyngitis und Infektion der oberen Atemwege (gewöhnliche Erkältung), Husten, Schwindel und Fatigue (Müdigkeit). Unter SEL wurden auffällige Leberwerte beobachtet.

Patienten werden nach dem Zufallsprinzip 2:2:1 einer der folgenden drei Behandlungsgruppen zugeteilt (randomisiert):
• Gruppe A: 240 Wochen Selonsertib 6 mg-Tablette + Placebo 18 mg-Tablette

• Gruppe B: 240 Wochen Selonsertib 18 mg-Tablette + Placebo 6 mg-Tablette
einmal täglich
• Gruppe C: 240 Wochen Placebo 6 mg-Tablette + Placebo 18 mg-Tablette einmal

Einschlusskriterien
- Leberbiopsie im Einklang mit NASH und Überbrückung (F3 Fibrose) nach der
NASH CRN Klassifizierung nach Ansicht des zentralen Lesers
- Hat beim Laborbesuch folgende Laborparameter:
* Alanin-Aminotransferase (ALT) ≤ 8 x obere Grenze der normalen (ULN)
* Kreatinin-Clearance (CLcr) ≥ 30 Milliliter / Minute (mL / min), berechnet
nach der Cockcroft-Gault-Gleichung
* Hämoglobin A1c (HbA1c) <9,5% oder Serum Fructosamin ≤ 381 μmol
- Gesamt Bilirubin ≤ 1,3 x ULN (ausser wenn andere Ursache bekannt)

Ausschlusskriterien
- Vorgeschichte einer dekompensierten Lebererkrankung einschliesslich Aszites,
Leber-Enzephalopathie (HE) oder Varizenblutung
- Child-Pugh (CP) Score> 6 am Screening ausser wenn andere Ursache bekannt
- Modell für Endstadium Lebererkrankung (MELD) Score> 12 beim Screening ausser wenn andere Ursache bekannt
- Andere Ursachen der Lebererkrankung, einschliesslich, aber nicht beschränkt
auf alkoholische Lebererkrankungen, Hepatitis B, Hepatitis C,
Autoimmunerkrankungen, drogeninduzierte Hepatotoxizität, Wilson-Krankheit,
Eisenüberladung und Alpha-1-Antitryspin-Mangel, basierend auf der Anamnese
und der zentralisierten Überprüfung der Leberhistologie.
- Geschichte einer Lebertransplantation
- Aktuelles oder Geschichte eines hepatozellulären Karzinoms (HCC)

Key Inclusion Criteria:
- Liver biopsy consistent with NASH and bridging (F3 fibrosis) according to the NASH
Clinical Research Network (CRN) classification in the opinion of the central reader
- Has the following laboratory parameters at the screening visit:
- Alanine aminotransferase (ALT) = 8 x upper limit of normal (ULN)
- Creatinine Clearance (CLcr) = 30 milliliter/minute (mL/min), as calculated by the
Cockcroft-Gault equation
- Hemoglobin A1c (HbA1c) = 9.5% (or serum fructosamine = 381 µmol if HbA1c is
unable to be resulted)
- Total bilirubin = 1.3 x ULN (unless an alternate etiology such as Gilbert's
syndrome or hemolytic anemia is present)
Key Exclusion Criteria:
- Prior history of decompensated liver disease including clinical ascites, hepatic
encephalopathy (HE), or variceal bleeding
- Child-Pugh (CP) score > 6, as determined at screening, unless due to therapeutic
anti-coagulation
- Model for End-stage Liver Disease (MELD) score > 12, as determined at screening,
unless due to therapeutic anti-coagulation
- Other causes of liver disease including, but not limited to, alcoholic liver disease,
hepatitis B, hepatitis C, autoimmune disorders, drug-induced hepatotoxicity, Wilson
disease, iron overload, and alpha-1-antitryspin deficiency, based on medical history
and/ or centralized review of liver histology.
- History of liver transplantation
- Current or history of hepatocellular carcinoma (HCC)
