Klinische Studie zur individuellen Behandlung von älteren Patienten mit neu diagnostiziertem primärem Lymphom des zentralen Nervensystems

- Die Behandlung in Teil A ist Patienten vorbehalten, die eine hochdosierte Chemotherapie erhalten können; sie besteht aus einer Induktionsphase, in der Sie drei Medikamente erhalten (Hochdosis-Methotrexat, Rituximab und Procarbazin), sowie aus einer anschliessenden Erhaltungsphase, in der Sie eines der folgenden beiden oralen Medikamente erhalten: Revlimid oder Procarbazin. Die Induktionsphase besteht aus 2 Behandlungszyklen mit einer Dauer von je 43 Tagen. Die Patienten, die am Ende der Induktion ein vollständiges Ansprechen (Verschwinden des Lymphoms), teilweises Ansprechen oder einen stabilen Zustand der Erkrankung erreichen, bekommen eine Erhaltungstherapie (Zweiter Teil der Behandlung) mit entweder Revlimid oder Procarbazin. Bei beiden handelt es sich um eine orale Therapie (Kapseln). Die Therapie mit Revlimid dauert 2 Jahre und die mit Procarbazin 6 Monate. Ob Sie die eine oder andere Therapie erhalten, wird per Zufallsverfahren entschieden. Nach der Erhaltungsphase folgt eine Beobachtungsphase über 2 Jahre ab Beginn der Erhaltungstherapie. - Die Behandlung in Teil B ist Patienten vorbehalten, die keine hochdosierte Chemotherapie erhalten können und stattdessen eine Kombination aus Radiotherapie und zwei Medikamnten, Rituximab und Temozolomid, erhalten. Teil B der Studie umfasst: -Eine Induktionsphase (Erster Teil der Behandlung) bestehend aus einem einzigen Behandlungszyklus mit Strahlentherapie, Rituximab und Temozolomid über einen Zeitraum von 28 Tagen. -Eine Erhaltungsphase (Zweiter Teil der Behandlung) mit Temozolomid, die 1 Jahr dauert. Es handelt sich um eine orale Therapie (Kapseln). Die Patienten bleiben maximal 2,5 Jahre in der Studie.

Im Rahmen der Studie werden körperliche, laboranalytische (Blutentnahmen) sowie apparative Untersuchungen (MRI des Schädels, CT Untersuchung) durchgeführt, die bei der Krankheit Behandlung üblich sind. Des Weiteren gibt es noch Tests um die geistigen Fähigkeiten zu testen (Neurokognition) sowie Fragebögen zu der Lebensqualität.

Criteri per la partecipazione alla sperimentazione (Fonte di dati: BASEC)

- Histologische oder zytologische Diagnose eines diffusen großzelligen B CD20+ Lymphoms
- Lymphom ausschließlich im zentralen Nervensystem lokalisiert (zerebrales und/oder meningeales Parenchym und/oder Diffusion in zerebrospinaler Flüssigkeit und/oder Augen und/oder Hirnnerven).
- Patienten im Alter von ≥70, die nicht zuvor behandelt wurden (zuvor oder aktuell zugelassene Steroidtherapie)

Criteri di esclusione (Fonte di dati: BASEC)

- Lymphom unterscheidet sich von dem diffundierten zu großen B-Zellen und extra ZNS Erkrankungen
- HIV oder Immunschwäche
- Überempfindlichkeit gegen einen Wirkstoff und/oder einen der sonstigen Bestandteile

Minimum age: 70 Years

Inclusion Criteria:

- Histologically or cytologically assessed diagnosis of CD20+ diffuse large B-cell
lymphoma.

- Diagnostic sample obtained by stereotactic or surgical biopsy, CSF cytology
examination or vitrectomy.

- Lymphoma exclusively localized in the central nervous system (brain parenchyma and/or
meningeal/CSF dissemination and/or eyes and/or cranial nerves).

- Previously untreated patients (previous or ongoing steroid therapy admitted).

- Age =70 years

- Patients not eligible for high-dose chemotherapy supported by autologous stem cell
transplant

- ECOG PS =3.

- Adequate bone marrow, cardiac, renal, and hepatic function

- No previous or concurrent malignancies with the exception of surgically cured
carcinoma in-situ of the cervix, carcinoma of the skin or other cancers without
evidence of disease at least for 3 years (patients with a previous lymphoma at any
time are NOT eligible).

- Absence of any familial, sociological or geographical condition potentially hampering
compliance with the study protocol and follow-up schedule.

- No concurrent treatment with other experimental drugs.

- Patients receiving oral lenalidomide or procarbazine must agree to avoid sharing the
study medication with another person and to return all unused study drug to the
investigator.

- Male patients must agree to always use a latex or synthetic condom during any sexual
contact with females of reproductive potential while taking lenalidomide, during dose
interruptions and for up to 7 days after treatment discontinuation, even if they have
undergone a successful vasectomy.

- Informed consent from the patient, or legal representative, obtained before
registration.

Exclusion Criteria:

- Lymphoma entity other than diffuse large B-cell lymphoma.

- Extra-CNS disease.

- Lymphoma exclusively localized in the eyes

- Lymphoma infiltration of the cranial nerves as exclusive site of disease

- Previous antineoplastic treatment for the PCNSL.

- Patients eligible for ASCT.

- HBsAg- and HCV-positive patients; HBsAg- and HCV-positive patients. HBcAb+ is not
exclusion criteria in the absence of detectable levels HBVDNA.

- HIV disease or immunodeficiency.

- Severe concomitant illnesses/medical conditions (e.g. impaired respiratory and/or
cardiac function, uncontrolled diabetes mellitus despite optimal medical management).

- Active infectious disease.

- Hypersensitivity to any active principle and/or any excipient according to the
contraindications reported in the Summary of Product Characteristics (SmPCs) of the
anticancer drugs used in the study
