Diese Studie wird etwa dreieinhalb Jahre dauern. Insgesamt werden etwa 72 Patienten an bis zu 50 Prüfzentren weltweit in die Studie eingeschlossen. Die Studie umfasst verschiedene Phasen. Während der ersten sechs Monate werden die Patienten nach dem Zufallsprinzip einer von zwei Behandlungsgruppen zugeteilt und erhalten einmal im Monat entweder Givosiran (in einer Dosis von 2,5 mg/kg) oder Placebo. Nach Abschluss der sechs Monate können die Patienten Givosiran unter Umständen monatlich für bis zu weitere 30 Monate erhalten.

• Voruntersuchungsphase (bis zu 2 Monate): Sobald Sie sich für eine Teilnahme entschieden und die Einwilligungserklärung unterzeichnet haben, beginnen Sie mit der Voruntersuchungsphase. Während dieser Phase wird bei Ihnen eine Reihe von Untersuchungen durchgeführt, um festzustellen, ob Sie die Voraussetzungen für die Studie erfüllen. Wenn Sie innerhalb von einer Woche vor einem Voruntersuchungstermin eine Normosang®-Infusion (Hämin) erhalten haben oder eine Porphyrie-Attacke hatten, werden Sie möglicherweise gebeten, während der Voruntersuchungsphase noch einmal eine Urinprobe abzugeben.
• Doppelblinde Behandlungsphase (6 Monate): Sobald der Prüfarzt festgestellt hat, dass Sie für die Studie infrage kommen, und sofern Sie die Teilnahme fortsetzen möchten, werden Sie in die Studie aufgenommen und für 6 Monate einmal im Monat mit der Prüfmedikation behandelt. Sie könnten als Prüfmedikation entweder Givosiran oder Placebo erhalten. Die Prüfmedikation wird in einer Dosis von 2,5 mg/kg als Injektion verabreicht. Das Placebo ist eine Kochsalzlösung, die genauso aussieht wie das Prüfpräparat. Doppelblind bedeutet, dass während dieser Phase weder Sie selbst noch der Prüfarzt wissen werden, ob Sie Givosiran oder Placebo erhalten. Ob Sie Givosiran oder Placebo erhalten, wird nach dem Zufallsprinzip (wie beim Werfen einer Münze) entschieden. Die Wahrscheinlichkeit, dass Sie während dieser Phase entweder Givosiran oder Placebo erhalten, liegt bei 1:1 (50 %).
• Unverblindete Verlängerungsphase (bis zu 29 Monate): Nachdem Sie die doppelblinde Behandlungsphase abgeschlossen haben, können Sie Givosiran monatlich für bis zu weitere 30 Behandlungsmonate erhalten oder bis der Sponsor die Entwicklung von Givosiran einstellt. Unverblindet bedeutet, dass sowohl Sie selbst als auch der Prüfarzt wissen werden, dass Sie während dieser Phase Givosiran erhalten. Wenn Sie beschliessen, während der doppelblinden Behandlungsphase aus der Studie auszuscheiden, können Sie nicht an der unverblindeten Verlängerungsphase teilnehmen.
• Nachbeobachtungsphase (1 Monate): Nachdem Sie die Behandlung abgeschlossen haben, werden Sie für einen Zeitraum von 3 Monaten nach der letzten Dosis des Prüfpräparats weiterhin beobachtet. Während der Nachbeobachtung werden weitere Bewertungen durchgeführt, um die Wirkungen des Prüfpräparats auf Ihre Erkrankung und die bei Ihnen aufgetretenen Nebenwirkungen zu untersuchen.

2) Dokumentierte Diagnose von Acute Hepatic Porphyria (Acute Intermittent
Porphyria, Hereditary Corproporhyria, Variegate Porphyria, ALA
dehydratase deficient porphyria)
3) Aktive Erkrankung mit mindestens 2 Porphyrie-Attacken innerhalb der
letzten 6 Monate vor dem Screening, die eine Hospitalisierung, eine
medizinische Notfallbehandlung oder eine i.v. Häminbehandlung zu Hause
erforderten.

Inclusion Criteria:
- = 12 years of age
- Diagnosed with Acute Hepatic Porphyria (Acute Intermittent Porphyria, Hereditary
Corproporhyria, Variegate Porphyria, aminolevulinic acid (ALA) dehydratase deficient
porphyria)
- Elevated urinary or plasma porphobilinogen (PBG) or ALA values within the past year,
- Have active disease, with at least 2 documented porphyria attacks within the last 6
months
- Willing to discontinue or not initiate the use of prophylactic hemin throughout the
study.
- Women of child bearing potential must have a negative serum pregnancy test, not be
nursing, and use acceptable contraception
Exclusion Criteria:
- Clinically significant abnormal laboratory results
- Anticipated liver transplantation
- History of multiple drug allergies or intolerance to subcutaneous injections
- Active HIV, hepatitis C virus, or hepatitis B virus infection(s)
- History of recurrent pancreatitis
