L’artériopathie des membres inférieurs est une maladie avec dépôts d’athérome (graisses, calcium et cellules inflammatoires) dans la paroi des artères des membres inférieurs. Ces dépôts peuvent provoquer des resserrements ou des obstructions des artères situées à la hauteur des cuisses ou des genoux (les artères fémorales superficielles et poplitées). La désobstruction de ces lésions par angioplastie et éventuelle pose de stent peut soulager les patients de leurs symptômes. L’angioplastie consiste à introduire un ballon dans la partie resserrée de l’artère, au moyen d’un cathéter, de gonfler le ballon pour élargir le passage du sang et de le retirer ensuite. L’angioplastie par ballon peut se compliquer de dissections (déchirure irrégulière) des dépôts d’athérome, avec persistance d’un mauvais passage du sang et nécessitant le placement d’un stent, afin de rétablir un passage régulier du sang dans l’artère. Le concept de préparation des lésions consiste à effectuer des fentes régulières et longitudinales dans la plaque d’athérome au moyen d’un cathéter particulier, avant de pratiquer l’angioplastie, afin d’obtenir un résultat régulier et harmonieux de l’angioplastie. On obtient ainsi un meilleur passage du sang, et le placement de stent est moins souvent nécessaire. Les angioplasties des artères fémorales et poplités sont faites de routine avec des ballons recouverts d’un médicament, le paclitaxel. On fait également l’hypothèse que la préparation de l’artère améliore la diffusion du médicament (paclitaxel) vers les couches de la paroi artérielle, là où il agit en freinant le resserrement qu’on peut parfois observer dans les mois qui suivent une angioplastie.
Cette étude prospective, a pour objectif d’évaluer les bénéfices à 1 an de la préparation artérielle par fentes longitudinales avant un traitement conventionnel par angioplastie au ballon à élution de paclitaxel, sur des sténoses ou occlusions aathéromateuses des artères fémorales superficielles ou poplités.

Criteria for participation in trial

• Patients adultes avec artériopathie oblitérante des membres inférieurs
• symptômes allant de la douleur musculaire lors de la marche, jusqu’à la gangrène de petite taille (limitée aux orteils)
• resserrement prouvé de ≥ 70 % au moins sur l’artère fémorale superficielle ou poplité
• Volonté de participer au protocole et signature du consentement
Exclusion criteria

• Femme enceinte, allaitante ou prévoyant une grossesse pendant la durée de l’étude
• Thrombose artérielle récente, donc friable, à risque de migrer au cours de l’intervention
• Utilisation d’un ballon à élution de paclitaxel dans la lésion au cours des 15 derniers mois
• Lésion très calcifiées, et étendues
Inclusion/Exclusion Criteria

Inclusion Criteria:
- Patients = 18-year-old, with symptomatic lower extremity occlusive arterial disease
- Rutherford class of symptom 2 to 5
- Atheromatous = 70 % stenosis or occlusion of SFA and/or PA
- Absence of > 50% residual stenosis of the run-in vessels at the end of procedure
- Presence of at least one patent (no = 50 % stenosis) run-off tibial or fibular vessel
to the foot at the end of procedure
- De novo or restenotic lesions, including in-stent restenosis
- Willingness to participate in the study and signature of informed consent
Exclusion Criteria:
- Renal failure with glomerular filtration rate (GFR) 30 ml/min or below, estimated by
the Cockroft-Gault equation.
- Women who are pregnant, lactating, or planning to become pregnant during the duration
of the study
- Recent artery thrombosis, at risk of distal embolization during percutaneous procedure
- Previous use of a PCB in the lesion during last 15 months
- Extremely calcified lesions, defined by calcification involving ³ 270° of the artery
circumference over ³ 15 cm.
- Sub-intimal recanalization
- Tortuous contra-lateral femoral access with difficult cross-over
- Previous or planned surgery of the target lesion
- High risk of bleeding
- Contra-indication to dual antiplatelet therapy for one month, and/or to single
antiplatelet therapy for 1 year
- Allergy to aspirin, clopidogrel or heparin
- Life expectancy less than one year
