Brief description of trial (Data source: BASEC)
Cette étude de phase 2, multicentrique, ouverte, randomisée et contrôlée est conçue pour évaluer l’innocuité et l’efficacité du pegcetacoplan chez des patients présentant une récidive post-transplantation de la glomérulopathie à dépôts de C3 (GC3) ou de la glomérulonéphrite membranoproliférative à complexes immuns (Ig-GNMP). Le pegcetacoplan agit en désactivant le système du complément, une partie du système immunitaire qui est suractivée chez les personnes atteintes de GC3 ou d'Ig-GNMP et qui endommage les reins. Dans des conditions normales, le système du complément aide à éliminer les agents pathogènes et les cellules endommagées sans entraîner de lésions dans l’organisme. Cependant, une activation parfois incontrôlée ou incorrecte du système du complément peut nuire à l’organisme.
Jusqu’à 12 adultes auxquels une récidive post-transplantation de GC3 ou d’Ig-GNMP a été diagnostiquée seront recrutés dans cette étude, dans 25 centres d’études à travers le monde. Cette étude comprend trois phases :
• Une période de sélection de 8 semaines maximum
• Une période d’étude principale de 52 semaines maximum
• Une période de suivi de 8 semaines maximum
Au cours des 12 premières semaines de l'étude, les participants seront affectés à des groupes de manière aléatoire (comme à pile ou face) pour recevoir ou non du pegcetacoplan. Après la semaine 12 de l’étude, tous les participants recevront le pegcetacoplan pendant toute la durée restante de l’étude. Le pegcetacoplan est administré à domicile deux fois par semaine en perfusion sous-cutanée (sous la peau). Chaque perfusion implique l'utilisation de 2 aiguilles à quelques centimètres l'une de l'autre dans 1 zone du corps et d'une pompe à perfusion munie d’une seringue afin de permettre l'écoulement du pegcetacoplan.
Tout patient qui, de l'avis de l'investigateur, présente un bénéfice clinique lié à l'administration de pegcetacoplan, peut participer à la partie B, une prolongation à long terme.

Patients âgés d'au moins 18 ans au moment de la sélection
2. Les patients doivent présenter des preuves cliniques et pathologiques de récidive de GC3 ou d'Ig-GNMP, comme en témoignent tous les éléments suivants :
a. Un diagnostic de GC3 ou d’Ig-GNMP, avec au moins 2+ de coloration pour C3c dans l'allogreffe rénale, confirmé par un pathologiste du centre, d’après la biopsie d'allogreffe rénale de la sélection
b. La GC3 ou l’Ig-GNMP doit être primaire et non secondaire à une autre affection (p.ex., infection, tumeur maligne, gammapathie monoclonale, auto-immunité, rejet chronique médié par les anticorps, microangiopathie thrombotique chronique ou prise d’un médicament)
3. Maladie stable (ne s'améliorant pas) ou s’aggravant, de l'avis de l'investigateur, dans les 2 mois précédant la première dose de pegcetacoplan
Exclusion criteria

1. Nombre absolu de neutrophiles <1000 cellules/mm3 pendant la sélection (sans compter le Jour 1)
2. Traitement antérieur par pegcetacoplan
3. Preuve de rejet, d’après la biopsie d'allogreffe rénale de sélection, nécessitant un traitement
Inclusion/Exclusion Criteria

Inclusion Criteria:
- Patients of at least 18 years of age at screening
- Patients must have clinical and pathologic evidence of recurrent C3G or IC-MPGN, as
evidenced by all of the following:
- Stable (not improving) or worsening disease, in the opinion of the investigator, in
the 2 months preceding the first dose of pegcetacoplan
- eGFR =30 mL/min/1.73 m2, calculated by the Chronic Kidney Disease-Epidemiology
Collaboration (CKD-EPI) creatinine equation for adults
- No more than 50% glomerulosclerosis or interstitial fibrosis on the screening renal
allograft biopsy
- Stable regimen for recurrent C3G/IC-MPGN for at least 4 weeks prior to the screening
renal allograft biopsy and from the time of the screening renal allograft biopsy until
randomization
- Willing to receive vaccinations against Neisseria meningitidis, Streptococcus
pneumoniae, and Haemophilus influenzae if applicable vaccination records are not
available
Exclusion Criteria:
- Previous treatment with pegcetacoplan
- Evidence of rejection on the screening renal allograft biopsy that requires treatment
- Diagnosis or history of HIV, hepatitis B, or hepatitis C infection or positive
serology at screening indicative of infection
- Malignancy, except for the following:
- Cured basal or squamous cell skin cancer
- Curatively treated in situ disease
- Malignancy free and off treatment for =5 years
- Significant renal disease in the renal allograft secondary to another condition (eg,
infection, malignancy, monoclonal gammopathy, rejection, or a medication) that would
confound interpretation of the study results
- Participation in any other investigational drug trial or exposure to other
investigational agent, device, or procedure within 30 days or 5 half-lives from the
last dose of the investigational agent (whichever is longer) prior to screening
