Brief description of trial (Data source: BASEC)
HALT est un essai clinique mené pour déterminer si un certain type de radiothérapie, appelé radiothérapie stéréotaxique corporelle (stereotactic body radiotherapy, SBRT) et qui permet de cibler très précisément les cellules cancéreuses lors de la radiothérapie, peut permettre d’augmenter la durée pendant laquelle des patients atteints de certaines formes de cancer du poumon avancé tirent profit de leur traitement actuel à base de médicaments appelés inhibiteurs de la tyrosine kinase (tyrosine kinase inhibitors, TKI).

Certains patients atteints d’un cancer du poumon peuvent voir se développer des modifications de la structure de certains de leurs gènes. Ces modifications sont appelées mutations et peuvent être des facteurs déterminants de leur maladie. Nous savons par exemple que des mutations peuvent survenir au niveau des gènes EGFR (récepteur du facteur de croissance épidermique) ou ALK (kinase lymphome anaplasique) et que les tumeurs qui présentent des mutations de ces gènes répondent mieux (comparativement aux tumeurs sans mutation) aux médicaments appelés inhibiteurs de la tyrosine kinase (TKI) qu’à une chimiothérapie standard.
Les TKI sont utilisés pour contrôler ou arrêter la propagation du cancer, mais ils finissent par cesser de fonctionner et les sites existants du cancer s’étendent et/ou se propagent à d’autres parties du corps. Lorsque cela se produit, on dit alors que le cancer « progresse ».
HALT étudie un traitement appelé radiothérapie stéréotaxique corporelle (SBRT). Nous souhaitons déterminer si l’utilisation de ce traitement peut permettre d’augmenter la durée pendant laquelle un patient tire profit du traitement par TKI.

Criteria for participation in trial

Patient de sexe masculin ou féminin de ≥ 18 ans (16 ans au Royaume-Uni)
Diagnostic histologique confirmé de CPNPC avancé, non éligible à un traitement radical, avec mutation définie exploitable recevant un traitement ciblé par TKI
Réponse clinique et/ou radiologiquement confirmée au traitement par TKI (évaluée localement, généralement 2 à 3 mois après avoir commencé le traitement par TKI)
Exclusion criteria

> 3 sites de maladie progressive extra-crânienne
Métastases cérébrales non éligibles à une chirurgie radicale ou à une radiochirurgie stéréotaxique
Radiothérapie antérieure à proximité de la lésion oligoprogressive excluant la possibilité d’une SBRT ablative. Les caractéristiques des lésions éligibles à une SBRT ablative dans le cadre de l’essai sont définies à la rubrique 4.1 du protocole et seront évaluées par l’équipe pluridisciplinaire virtuelle de HALT
Inclusion/Exclusion Criteria

Inclusion Criteria:
1. Male or female, = 16 years of age
2. Established histological diagnosis of advanced NSCLC, not suitable for radical
treatment, with defined actionable mutation receiving targeted TKI therapy
3. Clinical and/or radiologically confirmed response to TKI therapy (assessed locally
usually 2-3 months post commencing TKI)
4. Confirmed OPD defined as = 3 extracranial sites of progressive disease. All sites must
be visible, imaging defined targets and suitable for treatment with SBRT as determined
by the virtual multi-disciplinary team (MDT) and in accordance with the HALT
Radiotherapy planning and delivery guidance document.
5. Adequate baseline organ function to allow SBRT to all relevant targets
6. Predicted life expectancy = 6 months
7. Karnofsky Index = 60% and ECOG 0-2
8. Provision of written informed consent
Exclusion Criteria:
1. > 3 extracranial sites of progressive disease
2. Progressing or newly diagnosed brain metastases not amenable to radical surgery or
SRS. Treated brain metastases which have remained clinically and radiologically stable
for = 6 months are permissible.
3. Prior radiotherapy near the oligoprogressive lesion precluding ablative SBRT.
Suitability of lesions for ablative SBRT as part of the trial defined in section 4.1
of this document and will be determined by the HALT virtual MDT
4. Co-morbidities considered clinically precluding the safe use of SBRT (as detailed in
the HALT radiotherapy planning and delivery guidelines).
5. Any psychological, sociological or geographical issue potentially hampering compliance
with the study
