Brief description of trial (Data source: BASEC)
Dies ist eine Pilot-Studie, in welcher wir den Einsatz der Tc-99m-Tilmanocept (anti-CD206) Szintigraphie (SPECT) für die Visualisierung von CD206 positiven tumorassoziierten Makrophagen (TAM) in Melanomatumoren testen wollen. Die Studie findet im CHUV in Lausanne statt. Wir werden 20 Patienten einschliessen.

Die Studie untersucht den Einsatz der Tc-99m-Tilmanocept Szintigaphie (SPECT) für die Darstellung von CD206 positiven TAMs in Melanomtumoren. Die Resultate der CD206 Bildgebung werden mit den Resultaten der zuvor entnommenen klinischen Biopsien verglichen, welche ebenfalls für verschiedene Makrophagen Parameter mittels Immunhistochemie gefärbt werden. Die Resultate werden mittels qualitativer und quantitativer Statistik ermittelt.
In einer geplanten retrospektiven Analyse wollen wir die Korrelation zwischen dem Vorhandensein von CD206 positiven TAMs bei Melanommetastasen und dem Ansprechen der Patienten auf die Anti-PD1-Immuntherapie analysieren.

Drug: Tc-99m tilmanocept
Criteria for participation in trial

- weiblich und männlich
- Alter gleich oder über 18
- Diagnose: fortgeschrittenes Melanoma, Grad 3-4
- geplante anti-PD-1 Immunotherapie
- Einverständniserklärung erteilt
- FDG PET/CT innerhalb 4 Wochen vor der Teilnahme
- Gewebeprobe vorhanden
- mindestens 3 Tumore im FDG-PET entdeckt
Exclusion criteria

- Alter unter 18 Jahren
- Augenmelanom
- Schwangere oder stellende Frauen
- Frauen, welche eine Schwangerschaft planen während des Ablaufs dieser Studie
- frühere Teilnahme in dieser Studie
- Teilnahme des Investigators, seiner/Ihrer Familienmitglieder, Angestellte oder sonstige abhängige Personen
- frühere erhaltene Immunotherapie
- Anamnese einer Krankheit oder eines relevanten physischen oder psychiatrischen Zustands oder eines abnormalen physischen Befundes, die die Studienziele nach Einschätzung des Prüfers beeinträchtigen können
- Der Teilnehmer/die Teilnehmerin hat eine bekannte Allergie gegen Dextran-Exposition oder eine Nebenwirkung
- Unzureichende Kenntnisse der Projektsprache, Unfähigkeit, eine Einwilligung zu erteilen oder Verfahren zu befolgen
- Der Patient macht von seinem „Recht auf Nichtwissen“ Gebrauch und weigert sich, über zufällige Befunde informiert zu werden

Inclusion Criteria:
- Female and male
- Age = 18
- Diagnosis of having advanced melanoma , stage III-IV
- Scheduled for clinically indicated anti-PD-1 immunotherapy
- Informed Consent as documented by signature
- FDG PET/CT within 4 weeks before screening
- Biopsy available
- Minimum 3 lesions detected in FDG PET/CT
Exclusion Criteria:
- Age under 18
- Ocular melanoma
- Women who are pregnant or breast feeding,
- Intention to become pregnant during the course of the study,
- Previous enrolment into the current study,
- Enrolment of the investigator, his/her family members, employees and other dependent
persons,
- Previous immunotherapy,
- History of any disease or relevant physical or psychiatric condition or abnormal
physical finding which may interfere with the study objectives at the investigator
judgment
- The subject has a known allergy to or has had an adverse reaction to dextran exposure
- Insufficient knowledge of project language, inability to give consent or to follow
procedures
- The patient makes use of his/her "right not to know" and refuses to be informed about
incidental findings
