Brief description of trial (Data source: BASEC)
In dieser Studie wird das Wirksamkeits- und Sicherheitsprofil eines Prüfpräparats bei Männern und Frauen beurteilt, die an einer mit antineutrophilen zytoplasmatischen Antikörpern (ANCA-)assoziierten Vaskulitis, einschliesslich GPA und MPA, leiden. Diese Studie umfasst einen 2-wöchigen Voruntersuchungsabschnitt, einen 16-wöchigen Behandlungsabschnitt und einen 8-wöchigen Nachbeobachtungsabschnitt. Im Rahmen dieser Studie werden 14 Besuchstermine über einen Zeitraum von etwa 6 Monaten stattfinden.
Die Studienteilnehmenden werden nach dem Zufallsprinzip 1 von 3 Behandlungsgruppen zugewiesen. Die Studie ist in 2 Teile unterteilt. In Teil 1 der Studie beträgt die Wahrscheinlichkeit dafür, dass die Studienteilnehmenden IFX-1 erhalten, 50 %. Wenn grünes Licht für die Fortsetzung von Teil 2 der Studie gegeben wird, beträgt die Wahrscheinlichkeit dafür, dass die Studienteilnehmenden IFX-1 erhalten, im zweiten Teil der Studie ungefähr 70 %. Alle Studienteilnehmenden werden eine standardmässige Therapie zur Immunsuppression sowie eine der folgenden Kombinationstherapien erhalten (d. h. eine Therapie mit dem Prüfpräparat IFX-1 oder dem entsprechenden Placebo [Placebo-IFX-1] mit Glukokortikoiden (GK) in unterschiedlichen Dosen oder ohne GK): A) eine Kombinationstherapie mit dem Prüfpräparat IFX-1 und mit GK in einer verringerten Dosis, B) eine Kombinationstherapie mit einem dem Prüfpräparat entsprechenden Placebo (Placebo-IFX-1) und mit GK in der Standarddosis oder C) eine Kombinationstherapie mit dem Prüfpräparat IFX-1 ohne GK (Placebo-GK). Weder der/die Studienteilnehmende noch das Studienteam wird wissen, welche Behandlung der/die Studienteilnehmende erhält.
Das primäre Ziel besteht darin, die Wirksamkeit der Behandlung mit IFX-1 als Ersatz für die GK-Therapie bei Patienten mit GPA und MPA zu evaluieren.
Es werden voraussichtlich bis zu 81 Männer und Frauen mit GPA und MPA auf der ganzen Welt an dieser Studie teilnehmen.

Standardtherapie zur Immunsuppression:
Während der Remissionseinleitung: Rituximab oder Cyclophosphamid; wird nach Ermessen des Prüfarztes und gemäss den internationalen Behandlungsrichtlinien, dem lokalen Versorgungsstandard und der aktuellen Zusammenfassung der Produktmerkmale verabreicht
Während der Remissionserhaltung: Rituximab (RTX) oder Cyclophosphamid (CYC) oder Wechsel zu Azathioprin (AZA), Methotrexat (MTX), Mycophenolat-Mofetil (MMP) oder Mycophenolat-Natrium (MPN); wird nach Ermessen des Prüfarztes und gemäss den internationalen Behandlungsrichtlinien, dem lokalen Versorgungsstandard und der aktuellen Zusammenfassung der Produktmerkmale verabreicht
3 Behandlungsgruppen:
• IFX-1 + verringerte GK-Dosis (VGK)
• Placebo-IFX-1 + GK-Standarddosis (SGK)

Criteria for participation in trial

• Männliche und weibliche Patienten im Alter von mindestens 18 Jahren
• Patienten mit diagnostizierter GPA oder MPA gemäss den Definitionen der Chapel-Hill-Konsensuskonferenz (CHCC)
• Patienten mit einer neu diagnostizierten oder rezidivierenden GPA oder MPA, die eine Behandlung mit Rituximab oder Cyclophosphamid plus Glukokortikoiden in der Standarddosis erforderlich macht
Exclusion criteria

• Patienten mit irgendeiner anderen multisystemischen Autoimmunerkrankung
• Patienten, die innerhalb von 12 Wochen vor der Voruntersuchung eine Behandlung mit CYC oder RTX erhalten haben; falls der Patient zum Zeitpunkt der Voruntersuchung AZA, MMF, MPN oder MTX erhält, müssen diese Arzneimittel vor der Gabe der ersten Dosis CYC oder RTX abgesetzt werden
• Patienten, die innerhalb von 4 Wochen vor der Voruntersuchung eine Behandlung mit intravenös zu verabreichenden GK in einer kumulativen Dosis von > 3 g erhalten haben
• Patienten, die über einen Zeitraum von mehr als 6 Wochen vor der Voruntersuchung eine kontinuierliche Behandlung mit einem oral einzunehmenden GK in einer täglichen Dosis von > 10 mg Prednison-Äquivalent erhalten haben

Inclusion Criteria:
- Diagnosis of GPA or MPA
- Have = 1 major item, or = 3 other items, or = 2 renal items on the Birmingham
Vasculitis Activity Score Version 3 (BVASv3).
- Newly diagnosed or relapsed GPA or MPA that requires treatment with CYC or RTX plus
GCs.
- Glomerular filtration rate = 20 mL/min/1.73 m².
Exclusion Criteria:
- Any other multi-system autoimmune disease.
- Require mechanical ventilation at screening.
- Known hypersensitivity to any investigational medicinal product and/or any excipient.
- Rare hereditary problems of galactose intolerance, total lactase deficiency or
glucose-galactose malabsorption.
- Have required management of infections, as follows (a) Chronic infection requiring
anti-infective therapy within 3 months before screening. (b) Use of intravenous
antibacterials, antivirals, anti-fungals, or anti-parasitic agents within 30 days of
screening
- Current and/or history (within the previous 5 years) of drug and/or alcohol abuse
and/or dependence.
- Evidence of Hep B, C and/ or HIV infection. Only subjects with documented negative
historical results (within 4 weeks before screening) for Hep B,C Virus and HIV or a
negative test by Screening can be included into the study.
- Abnormal laboratory findings at screening
- Current or history of malignancy, lymphoproliferative, or myeloproliferative disorder
- Received CYC or RTX within 12 weeks before screening or within 12 weeks before CYC or
RTX is started for remission induction within 2 weeks before screening.
- Received > 3 g cumulative intravenous GCs within 4 weeks before screening.
- Received an oral daily dose of a GC of > 10 mg prednisone-equivalent for more than 6
weeks continuously prior to screening.
- Received an oral daily dose of a GC of > 80 mg prednisone equivalent within 2 weeks
before screening.
- Received a CD20 inhibitor, anti-tumor necrosis factor treatment, abatacept,
alemtuzumab, any other experimental or biological therapy, intravenous immunoglobulin
(Ig) or plasma exchange, antithymocyte globulin, or required renal dialysis within 12
weeks before screening.
- Received a live vaccination within 4 weeks before screening
- Either active or latent tuberculosis treatment is ongoing.
- Pregnant or lactating.
- Abnormal electrocardiogram.
- Female subjects of childbearing potential unwilling or unable to use a highly
effective method of contraception
- Participation in an investigational clinical study during the 12 weeks before
screening.
- Male subjects with female partners of childbearing potential unwilling to use
contraception
