Brief description of trial (Data source: BASEC)
Dies ist eine zweiteilige Studie. Am Teil 1 der Studie werden rund 80 Patienten mit Typ 1 Hepatitis teilnehmen, die auf die Standardtherapie nicht vollständig ansprechen oder diese nicht vertragen.
Nach einer vierwöchigen Voruntersuchungsphase werden die Patienten nach dem Zufallsprinzip für eine 52-wöchige Behandlungsphase einer von 4 möglichen Gruppen zugeteilt: 1: VAY736 hochdosiert (300mg), 2: VAY736 mitteldosiert (50mg), 3: VAY736 tiefdosiert (5mg) oder 4: Placebo. Weiterhin erhalten die Patienten Kortikosteroide und/oder Azathioprine.
Patienten der Placebogruppe erhalten das Placebo während den ersten 28 Wochen, anschliessend wechseln sie während 24 Wochen auf VAY736 mittel bis hochdosiert (150mg).
Die Patienten werden nach erfolgter Behandlung während mindestens 20 Wochen und bis zu 2 Jahre nachverfolgt.
Teil 1 der Studie soll zeigen ob VAY736 gegenüber Placebo überlegen ist. Dies wird bei den verschieden Gruppen primär anhand der Normalisierung des Leberwertes Aminotransferase (ALT) nach 24 Wochen gemessen.
Teil 2 der Studie startet erst nachdem die geeignete Dosierung von VAY736 mittels Teil 1 der Studie identifiziert wurde, mögliche Anpassungen im Studiendesign vorgenommen und die entsprechenden Genehmigungen der Behörden eingeholt wurden.

1. Teil 1: Männliche und weibliche Patienten/Patientinnen zwischen 18 und 75 Jahre mit Typ 1 Autoimmunhepatitis (AIH)
2. Leberbiospie zur Diagnose der AIH
3. Unvollständiges ansprechen oder Intoleranz gegenüber der Standardtherapie

2. Dekompensierte Zirrhose
3. Diagnose einer Autoimmunhepatitis mit Overlap-Syndrom (z.B. AIH & primär biliäre Zirrhose oder AIH & primär sklerosierende Cholangitis)

Key Inclusion Criteria:
1. AIH diagnosed per International Autoimmune Hepatitis Group
2. Liver biopsy with Ishak modified HAI indicating active AIH
3. Incomplete response to OR intolerance of standard therapy (per AASLD)
Key Exclusion Criteria
1. Prior use of any B-cell depleting therapy (e.g., rituximab or other anti-CD20 mAb,
anti-CD22 mAb or anti-CD52 mAb) within 1 year prior to Screening or as long as B-cell
count <50 cells/?L
2. Required regular use of medications with known hepatotoxicity
3. Decompensated cirrhosis
4. Diagnosis of overlap syndrome with AIH (e.g., AIH+PBC, AIH+PSC).
5. Drug related AIH at screening or a history of drug related AIH.
6. History of drug abuse or unhealthy alcohol use
7. History of malignancy of any organ system
8. Pregnant or nursing (lactating) women
