Brief description of trial (Data source: BASEC)
Bedingt durch die Erkrankung wird die Patientin/ der Patient in einen Schlaf ähnlichen Zustand versetzt (sediert). Die Sedation kann über Medikamente erfolgen, welche über die Vene verabreicht werden oder alternativ, durch ein über die Lunge eingeatmetes Narkosemittel.
Sobald die Patientin/der Patient in die Studie eingeschlossen ist, werden wir Sie/Ihn mittels eines Computers zufällig in zwei Behandlungsgruppe einteilen (randomisieren). Die Behandlungsarme sind zum einen die Gruppe welche das Narkosemittel Sevofluran über die Lunge bekommt «Sevo» und zum anderen die Gruppe, welche das herkömmliche Narkosemittel über die Vene erhält «Kontrollgruppe». Die Wahrscheinlichkeit in die Behandlungsgruppe «Sevo» eingeschlossen zu werden beträgt 50%, sodass wir bei Studienabschluss in beiden Gruppen die gleiche Anzahl Patientinnen/ Patienten haben.
Bei Patientinnen/ Patienten in der Gruppe «Sevo» werden wir die herkömmliche Sedation über die Vene stoppen und die Patienten während 48 Stunden über die Lunge mittels Sevofluran sedieren. Nach 48 Stunden wechseln wir wieder zur ursprünglichen Standardtherapie zurück.
In der «Kontrollgruppe» wird die Sedation gemäss der Standardtherapie der Intensivstation intravenös weitergeführt

Inclusion/Exclusion Criteria

Inclusion Criteria:
- SARS-CoV-2 infection (positive testing) or computed tomography (CT) scan-suspected
COVID-19 ARDS
- Male and female patients, age 18 to 85 years
- ICU patients with ARDS defined as PaO2/FiO2 < 200mmHg (=26.6kPa)
- Time of intubation not longer than 24 hours
- QTc Time (ECG) not longer than 470 ms ? (male)/ 480 ms ? (female)
- Sedation and mechanical ventilation in ICU
- Informed consent, signed by a representative or by an independent physician
Exclusion Criteria:
- High dose systemic corticosteroids in the phase before hospitalization (> 10mg/d
prednisone or equivalent dose)
- Significant concomitant disease (acute cerebral vascular event, acute coronary
syndrome, seizure, burn, neuromuscular disease)
- Organ transplant
- AIDS
- Pregnancy and/or breastfeeding
- Use of cytokine absorber
