Brief description of trial (Data source: BASEC)
In dieser Studie wollen wir herausfinden, ob mittels der neuen Lasertherapie die Symptome und Merkmale des Lichen sclerosus verbessert werden. Die Teilnehmerinnen werden nach dem Zufallsprinzip in zwei Gruppen aufgeteilt: in eine Gruppe, welche die Laser-Behandlung erhält (Lasergruppe, 2/3 aller Patientinnen) und eine andere Gruppe, die eine Kortisontherapie erhält (Kortisongruppe, 1/3 aller Patientinnen). Verlaufskontrollen erfolgen nach 6 und nach 12 Monaten. Der Kortisongruppe wird nach 6 Monaten auch die Laser-Behandlung offeriert.

Lichen sklerosus ist eine chronische Hauterkrankung, die mit extremem Juckreiz, wiederkehrendem Brennen, Schmerzen und Entzündungen im Genitalbereich verbunden ist.

Lasergruppe: 4 Lasersitzungen mit einem Zweifachlaser im Abstand von 4-8 Wochen.
Kortisongruppe: Standardtherapie mit hochdosierter Kortisonsalbe, mit schrittweiser Reduktion der Dosis alle 2 Monate.

Criteria for participation in trial

- Frauen älter als 18 Jahre alt mit der Diagnose Lichen sclerosus
- Einverstanden, dass zwei Biopsien durchgeführt werden
Exclusion criteria

- andere gleichzeitige Therapie von Lichen sclerosus (muss 2 Wochen vorher gestoppt werden)
- BMI >35
- Kontraindikationen gegenüber der geplanten Behandlungsgruppen (Kotison und Laser)

Inclusion criteria:
- Diagnosis of vulvar lichen sclerosus
- Clinical LS score = 4
Exclusion criteria:
- Concomitant steroid, calcineurin inhibitor or any other topical or systemic treatment
for LS
- < 3 months since start of vaginal estrogen treatment
- Malignant disease as the cause of the vulval symptoms
- BMI > 35 kg/m?
- Acute infection (fungal, bacterial, viral) of the vulva, vagina or bladder
- Presence of contraindications for the laser treatment or topical steroid treatment
