Brief description of trial (Data source: BASEC)
In vielen Notfallstationen behandeln wir Schmerzen hauptsächlich mit Paracetamol (z.B. Dafalgan®, Panadol®,…), starke Schmerzen mit Opioiden wie z.B. Morphin. Opioide können unangenehme Nebenwirkungen wie Übelkeit, Erbrechen und Schwindel verursachen.
Wir möchten mit unserer Studie untersuchen, ob bei Patienten einer Notfallstation die Kombination von Paracetamol und Morphin eine günstigere Wirkung mit weniger Nebenwirkungen zeigt. Durch die kombinierte Gabe beider Arzneimittel gehen wir von einem geringeren Bedarf an Morphin aus, die Nebenwirkungen sollten dadurch verringert werden. Bei frisch operierten Patienten konnte dies bereits in einer Untersuchung nachgewiesen werden.

Zum Vergleich der Schmerzlinderung werden die Patienten in zwei Gruppen einteilt. In einer Gruppe wird die Wirkung der Kombination von Paracetamol mit Morphin und in der anderen Gruppe die Wirkung von Morphin alleine getestet. Damit das Behandlungsteam (Pflege und Ärzte) objektiv bleibt, darf es nicht wissen, welche Schmerzmittel der Patient erhält (nur bezüglich Paracetamol). Dazu wird die Infusion des Studie (Paracetamol oder Kochsalzlösung) durch ein unabhängiges Team vorbereitet und unkenntlich gemacht. Auf diese Weise, weiss der Patient und das Behandlungsteam nicht, ob Morphin alleine oder in Kombination mit Paracetamol verabreicht worden ist. Morphin wird mit der Start-Dosis von 0.1 mg/kg begonnen. Die Schmerzmittelgabe wird alle 15 Minuten wiederholt (0.05 mg/kg) bis der Schmerz nachlässt oder starke Nebenwirkungen auftreten, die einen Wechsel auf andere Medikamente erfordern. Der Beobachtung der Schmerzlinderung stoppt 4 Stunden nach Verabreichung der ersten Infusion. Die Schmerzintensität wird mit einer Schmerzskala von 0-10 durch den Patienten festgelegt. Es werden insgesamt 200 Patienten in diese Studie aufgenommen.

Es können alle volljährigen Personen teilnehmen, die an Schmerzen leiden.
Exclusion criteria

Nicht teilnehmen dürfen Personen, die auf eine dauernde Einnahme von Schmerzmitteln angewiesen sind oder die bereits in den letzten 6 Stunden ein Schmerzmittel eingenommen haben. Patienten mit Allergie und/oder Unverträglichkeit auf Morphin oder Paracetamol sind ebenfalls ausgeschlossen. Ebenfalls, Patienten die eine Schmerzbehandlung mit Morphin und/oder Paracetamol sich nicht wünschen oder die bei welcher eine Behandlung in der Notfallpraxis geeignet ist.

Inclusion Criteria:
- Patient with pain with Pain-score is more or equal than four in a numeric rating scale
from zero to ten (i.e. indication for a treatment with morphine)
- Age over 18
- Signed informed consent. In case, patient is not able to sign the informed consent due
to pain, at least verbal consent has to be provided. After pain relief, written
informed consent with data and time has to be obtained
Exclusion Criteria:
- Analgesia in the past last 6 hours prior to visit at the emergency department
- Current Analgesia with long-acting/extended-release drugs
- Current Analgesia with opioids
- Chronic pain syndrome
- Contraindication for either paracetamol or morphine
- Patient's refusal of paracetamol or morphine treatment
- Pregnancy or Breastfeeding
- GCS < 13
- SpO2 < 90% with a maximum of 4 L/Min O2
- Systolic Blood Pressure < 90 mmHg
- Fast Track/Notfallpraxis patients
