Brief description of trial (Data source: BASEC)
Zur Behandlung einer Mitralklappeninsuffizienz gibt es derzeit verschiedene Möglichkeiten. Eine medikamentöse Behandlung, um die Symptome der Insuffizienz zu verringern. Ein chirurgischer (künstlicher) Mitralklappenersatz, oder aber es besteht die Möglichkeit die Mitralklappe mittels eines chirurgischen Eingriffs zu reparieren. In unserem Forschungsvorhaben wollen wir die Sicherheit und Wirksamkeit des SAPIEN M3-Systems ohne Kontrollgruppe bei Patienten mit symptomatischer Mitralinsuffizienz von Grad 3 oder höher, bei denen kommerziell erhältliche chirurgische oder Transkatheter-Behandlungsoptionen aus klinischen, anatomischen oder technischen Gründen als ungeeignet erachtet werden, untersuchen. Das SAPIEN M3-Systems ist in der Schweiz noch nicht zugelassen. Nach Einwilligung zur Teilnahme an der Studie werden ausführliche Voruntersuchungen erfolgen, um festzustellen, ob der Patient an der Studie teilnehmen kann. Danach erfolgt das Klappenersatz-Verfahren, bei dem eine künstliche Herzklappe über die Vene in der Leiste mit einem Katheter in die Mitralklappe des Herzens implantiert wird. Das Verfahren dauert voraussichtlich zwischen 1 und 3 Stunden. Zusätzlich zu den Visiten während des Krankenhausaufenthaltes wird der Patient gebeten nach 30 Tagen, nach 6 Monaten sowie jährlich, und zwar für 5 Jahre zu Nachuntersuchungen ins Krankenhaus zu kommen. Während der Visiten wird der Patient gebeten zwei Fragebögen zu beantworten und einige Untersuchungen (wie z. B. EKG, Blutabnahme, Echokardiografie des Herzens) durchführen zu lassen.

1. MR ≥3+
2. NYHA-Funktionsklasse ≥II
3. Laut Herzteam sind kommerziell erhältliche chirurgische oder
Transkatheter-Behandlungsoptionen aus klinischen, anatomischen
oder technischen Gründen als ungeeignet erachtet werden.
Exclusion criteria

1. Mitrale/kardiale Anatomie, die eine angemessene Einbringung und
Entfaltung des Docks oder der Klappe ausschließen würde
2. Zuvor erfolgte Herztransplantation
3. Myokardinfarkt in den letzten 30 Tagen vor dem Eingriff

Inclusion Criteria:
1. 18 years of age or older
2. MR = 3+
3. NYHA functional class = II
4. Per the Heart Team, commercially available surgical or transcatheter treatment options
are deemed unsuitable due to clinical, anatomic or technical considerations.
5. Subject's heart failure management has been optimized based on subject characteristics
and applicable guidelines, and stable for at least 30 days prior to enrollment.
6. The subject or subject's legal representative has been informed of the nature of the
study, agrees to its provisions and has provided written informed consent.
Exclusion Criteria:
1. Mitral/cardiac anatomy that would preclude appropriate delivery and deployment of the
SAPIEN M3 dock or valve
2. Inappropriate anatomy for femoral introduction and delivery of the SAPIEN M3 dock and
valve
3. Presence of any device that will contact or interfere with the SAPIEN M3 System during
delivery or after implantation
4. Left ventricular ejection fraction
