Brief description of trial (Data source: BASEC)
Der Zweck dieser klinischen Prüfung ist es die ideale Flüssigkeit (es gibt verschiedene Infusionslösungen mit unterschiedlicher Blutsalzzusammensetzung) für herzchirurgische Patienten zu finden, damit die Gabe von kreislaufunterstützenden Medikamenten so minimal wie möglich gehalten werden kann. Es gibt zu dieser Fragestellung sehr wenige klinische Untersuchungen, deshalb ist es wichtig herauszufinden, welche Infusionslösung in welcher Zusammensetzung am besten zur Behandlung von Herzkreislaufschwankungen während herzchirurgischen Operationen ist.

Es werden Patienten mit einer Erkrankung einer oder mehreren Herzklappen sowie Patienten mit einer Erkrankung der Herzklappen sowie den Herzkranzgefässen welche sich einer Herzklappenoperation unterziehen müssen untersucht.

Während und nach herzchirurgischen Operationen können aus verschiedenen Gründen Blutdruckschwankungen auftreten welche nach internationalen Standards zuerst mit der Gabe von Flüssigkeit (Infusionslösung) und falls nicht ausreichend mit kreislaufunterstützenden Medikamenten behandelt werden. Verglichen wird die Infusionslösung RingerLactat mit der Lösung RingerAcetat Infusionslösung. Beide Flüssigkeiten werden im Routinebetrieb seit Jahren verwendet und sind vom Bundesamt für Gesundheit zugelassen. Nebenwirkungen sind seit der Zulassung keine Beschrieben.

Criteria for participation in trial

In die Studie werden Patienten welche sich einer Operation der Herzklappen oder der Herzkranzgefässe und einer Herzklappen unterziehen eingeschlossen.
Exclusion criteria

Nicht eingeschlossen werden können Patienten mit schweren Vorerkrankungen (Niere, schwere Infektion) sowie schwangere oder stillende Patientinnen.
Inclusion/Exclusion Criteria

Inclusion Criteria:
- Elective single heart valve replacement
- Elective double valve replacement
- Elective single or double valve replacement and coronary artery bypass grafting
Exclusion Criteria:
- Patients unable to give informed consent
- Patients younger than 18 years of age or older than 80 years
- Pregnancy or breastfeeding
- Ejection fraction (EF) of less than 30% preoperatively
- Preexisting renal insufficiency with a glomerular filtration rate below 30ml/min
- Patients transferred form the intensive care unit to the operating theater
- Emergency operation
- Reoperation
- Patients planned for fast-track surgery
- Patients planned for minimal extracorporal circuits
- Preexisting anemia requiring immediate perioperative blood transfusion
- Chronic inflammatory diseases
- Any signs of infection or sepsis
- Limitation of full therapy (e.g. Jehowa's witnesses)
