Bei der Kontrolle von Tumoren spielt das Immunsystem eine wichtige Rolle.
Pembrolizumab ist ein Antikörper, der die Täuschung des Immunsystems durch den Tumor unterbindet und so dessen körpereigene Bekämpfung steigert.
Diese klinische Studie schliesst Patienten mit einem neu diagnostizierten, lokalfortgeschrittenem Tumor im Kopf-Hals-Bereich ein.
Das Ziel der Studie besteht darin, die Wirksamkeit von Pembrolizumab als Zusatz vor und nach der Operation zur zugelassenen Standard-Behandlung zu untersuchen und mit dieser zu vergleichen. Die Standardtherapie umfasst die Operation und vollständige Entfernung des Tumors mit anschliessender Bestrahlung mit oder ohne Chemotherapie (je nach gewissen Risikofaktoren).

Jährlich erkranken etwa 600‘000 Menschen weltweit an einem Tumor im Kopf-Hals-Bereich. Damit sind die Krebsarten in dieser Körperregion am siebenhäufigsten. In diese Studie werden Patienten mit neu diagnostiziertem, lokal-fortgeschrittenem Tumor des Typs Plattenepithelkarzinom im Kopf-Hals-Bereich einbezogen, für welche eine operative Entfernung empfohlen wird.
An insgesamt etwa 600 Patienten weltweit soll nachgewiesen werden, inwieweit die Anwendung von Pembrolizumab als zusätzliche Therapie zur Standardtherapie
gegenüber der Standardtherapie ohne Pembrolizumab die Effizienz der Behandlung verbessert.

Nach genauer Eignungsprüfung, Erhebung der Krankengeschichte und ausführlicher Aufklärung wird der Patient in die Studie eingeschlossen und nach dem Zufallsprinzip einem der beiden Behandlungsarme zugeordnet. Die Wahrscheinlichkeit der Zuteilung zu einer der beiden Gruppen ist gleich gross (je 50%).
Gruppe A erhält zwei Behandlungszyklen Pembrolizumab vor der Operation und 15
Behandlungszyklen Pembrolizumab nach der Operation. Die Patienten erhalten
ebenfalls Bestrahlung mit oder ohne Chemotherapie (Cisplatin, drei Zyklen) nach der Operation.
Gruppe B wird direkt operiert und wird nach der Operation Bestrahlung mit oder ohne Chemotherapie (Cisplatin, drei Zyklen) erhalten.
Die medikamentöse Therapie (Pembrolizumab, Chemotherapie) erfolgt in
Behandlungszyklen in 3-wöchigen Abständen, wobei die Medikamente am ersten
Tag des dreiwöchigen Therapiezyklus über die Vene per Infusion verabreicht
werden.
Nach der Operation wird anhand der Analyse der Tumor-Proben bestimmt, ob ein
Patient als Hochrisikopatient gilt oder nicht. Hochrisikopatienten erhalten nach der Operation eine höhere Strahlendosis und zusätzlich Chemotherapie, während Nicht-Hochrisikopatienten keine Chemotherapie und nur die Strahlentherapie erhalten.
Diejenigen Patienten, welche Pembrolizumab oder Chemotherapie erhalten, werden während ca. einem Jahr (Pembrolizumab) bzw. ca. 3 Monaten (Chemotherapie) alle 3 Wochen mittels Infusion behandelt. 30 Tage nach Behandlungsende wird eine Kontrollvisite durchgeführt. Wenn der Tumor nicht gewachsen ist, werden während 3 Jahren etwa alle 12 Wochen mittels Computer Tomographie Bilder erstellt, danach noch 2 Jahre lang alle 6 Monate. Nach dieser Zeit werden die Patienten ungefähr alle 12 Wochen oder häufiger kontaktiert, um den Gesundheitszustand abzufragen.
Im Rahmen der Studientermine wird eine körperliche Untersuchung zur
Dokumentation der Vitalzeichen durchgeführt. Darüber hinaus wird ein
Elektrokardiogramm (EKG) aufgezeichnet, Zähne und Mundhöhle untersucht, ein
Hörtest durchgeführt sowie neurologische Untersuchungen durchgeführt. Ausserdem werden Blut- und Urinproben gesammelt und bildgebende Untersuchungen mittels Computer-, Magnetresonanz- oder Positronen-Emissions- Tomographien (CT/MRI/PET) durchgeführt. Sollte eine Verschlechterung der Krankheit bei den bildgebenden Verfahren festgestellt werden, wird das weitere Vorgehen mit dem Patienten besprochen. Es wird ausserdem eine Gewebeprobe des Tumors gemacht und allenfalls auf Humanes Papilloma Virus (HPV) untersucht, falls der Tumor im hinteren Teil der Mundhöhle oder im Rachen liegt.
Die gesamte Studiendauer für den einzelnen Teilnehmer beträgt ca. 5 bis 6 Jahre.

Criteri per la partecipazione alla sperimentazione

1. Neu diagnostizierter histologisch bestätigter, lokal- fortgeschrittener,
operabler/nicht-metastasierter Tumor des Typs Plattenepithelkarzinom im
Kopf-Hals-Bereich (Stage III bis IVa).
2. Ein operativer Eingriff zur Entfernung des Tumors wird vom Arzt empfohlen
3. Der Patient weist eine nach Recist 1.1 auswertbare Tumorbelastung auf,
welche mittels Computertomographie (CT) oder Magnetresonanztomographie
beurteilt wurde. Die Tumorbelastung, welche mittels Computertomographie
(CT) oder Magnetresonanztomographie beurteilt wird, ist nach RECIST 1.1
auswertbar.

1. Tumore, welche schon weiter fortgeschritten sind und vielleicht schon
metastasiert sind.
2. Tumore ausserhalb des Mund-Rachen-Bereichs (Oropharynx), des
Kehlkopfes (Larynx), des untersten Teils des Rachens (Hypopharynx) oder
der Mundhöhle, wie zum Beispiel Tumore im Bereich des
Nasenrachenraumes (Nasopharynx) , der Nasennebenhöhlen (Sinus)
paranasale oder andere unbekannte primäre Tumore im Kopf-Hals-Bereich.
3. Tumore welche bereits mit einer Radiotherapie oder einer systemischen
antitumorale Therapie (inklusive Therapien im Rahmen klinischer Studien)
behandelt wurden.

Main Objective: All objectives for this study are exploratory:
- To investigate the long-term safety and tolerability of ponesimod.
- To investigate the long-term efficacy of ponesimod.
- To explore the dose response relationship of 10, 20 and 40 mg
ponesimod on lymphocyte count, MRI endpoints, annualized relapse rate (ARR), and safety endpoints.;Secondary Objective: Not applicable;Primary end point(s): Only exploratory efficacy endpoints will be investigated.
The following exploratory endpoints will be analyzed:
1. Annualized confirmed relapse rate.
2. Time to first confirmed relapse
3. Time to 24-week confirmed disability progression up to end of the study.
