Der Zweck dieser Studie ist die Effektivität eines mit Smartphone App begleiteten Geh-Trainings bei Patienten mit Diabetes Typ 2, oder einer Vorstufe davon, zu untersuchen und die Machbarkeit des Studiendesigns zu erforschen, um anschließend eine Studie in einem größeren Rahmen durchzuführen.
Die Kontrollgruppe wird dabei ein kontinuierliches Ausdauertraining absolvieren. Ausdauertraining in Form von Geh-Training gilt als Standardmethode bei der Behandlung von Patienten mit Diabetes Typ 2 und stellt daher kein erhöhtes Risiko für die Probanden dar.
Studien zeigen, dass Ausdauertraining in Intervall-Form effektiver ist, als kontinuierliches Ausdauertraining (Karstoft et al., 2013, 2014; Maillard, Pereira, & Boisseau, 2018). Eine einfache und dennoch sehr effektive Maßnahme ist ein individuelles Outdoor Geh-Training im Bezug auf eine Steigerung der Trainingskapazität und Lebensqualität der Menschen mit Diabetes Typ 2.
Smartphone Applikationen können dabei die Einhaltung und Durchführung des Intervall-Trainings verbessern und sie sind auch ein motivierendes Hilfsmittel für eine langanhaltenden Aktivitätssteigerung der betroffenen Personen (Brinkløv et al., 2016; Byrne et al., 2017; Glynn et al., 2014; Ried-Larsen et al., 2016; Valentiner et al., 2017; Walker, Valentiner, & Langberg, 2018). Daher könnte ein Smartphone App begleitetes Intervall-Training eine praktikable und preiswerte Lösung sein, um einen positiven Beitrag zur Bekämpfung der anstehenden Diabetes-Epidemie zu leisten.

Die Smartphone App begleitete Intervall Gruppe absolviert das Gehtraining 3 mal pro Woche für 4 Wochen. Die Intervall Blöcke sind folgendermaßen eingeteilt und werden abwechselnd insgesamt 30 min absolviert: 3 min schnelles Gehen (hohe Intensität), 3 min langsameres Gehen (niedrigere Intensität).
Die Kontrollgruppe absolviert ein kontinuierliches Ausdauertraining 30 min lang in einer moderaten Intensität 3 mal die Woche für 4 Wochen.
Die Trainingseinheiten werden jeweils 3 mal pro Woche ausgeführt. Dabei werden sie 2 mal pro Woche von zwei PhysiotherapeutInnen betreut und 1 mal pro Woche sollen sie das Training individuell ausführen (bevorzugt am Wochenende).

Group 1: The smartphone app assisted interval-walking group is going to complete a walking training of 30 minutes, 3 times per week for 4 weeks. The intervals contain blocks of 3 minutes of high intensity, followed by 3 minutes of lower intensity walking.
The training is guided by 2 physiotherapists 2 times per week and once per week the individuals have to perform the training by themselves (preferable on the weekend).
Group 2: The continuous walking group (control group) is going to walk 30 minutes at a medium pace 3 times per week for 4 weeks.

• Müssen 30 min kontinuierlich gehen können
• Keine Kontraindikationen, die sportliche Aktivitäten untersagen
Criteri di esclusione

• Menschen, die nicht fließend deutsch lesen und schreiben können
• Menschen, die unter 18 Jahren alt sind
• Kontraindikationen oder Verletzungen der unteren Extremitäten, die sportliche Aktivitäten untersagen
- Menschen, die Langzeitinsulin und Sulfonylharnstoffe bekommen

Inclusion criteria: Participants fulfilling all of the following inclusion criteria are eligible for the study:
? Informed Consent as documented by signature

? Individuals with type 2 diabetes or prediabetes diagnosed by a GP
? Ability to walk 30 min continuously?
? Ability to read and speak German ?fluently
? No contraindication, which prohibit physical activity
? Owning a smartphone (Apple / Android)
Exclusion criteria: The presence of any one of the following exclusion criteria will lead to exclusion of the participant: ?
? Inability to follow the procedures of the study, e.g. due to language problems, psychological disorders, dementia, etc.
? Patients receiving long-term insulin and/or sulfonylurea treatment
? Contraindication, which prohibit physical activity (e.g. uncontrolled arterial hypertension, limited cardiac insufficiency)
? Limiting musculoskeletal disorders, which prohibit the participation (Injury in lower limbs, painful osteoarthritis etc.)
? No informed consent
? <18 years of age

The primary outcome of this pilot study is to assess the feasibility of the study design that is key to the success of a large-scale study.
The feasibility factors of the study design are measured by reasons for exclusion, recruitment rate, reasons for dropouts, patients? satisfaction, intensity of the training method, missing data, estimated sample size. The feasibility factors will be measured during the whole study time and are assessed by descriptive statistics, questionnaires and interviews.

The secondary outcome is a comparison between the effects of a smartphone app guided outdoor interval walking-training and a continuous aerobic walking-training in terms of anthropometric measurements (WtHR, weight), aerobic capacity (6-MWT) and improvements in quality of life (SF-36) in patients with type 2 diabetes or prediabetes. These measurements will take place at baseline and after 4 weeks of training.
