Cette étude est destinée à évaluer si l’intervention précoce par administration sous-cutanée (s.c) de 300 mg de secukinumab chez des patients atteints de psoriasis en plaques modéré à sévère d’apparition récente peut permettre de prolonger les périodes sans symptômes en évitant la réactivation des lésions anciennes ou finalement en empêchant totalement l’apparition de nouvelles lésions, c’est-à-dire en modifiant l’évolution naturelle de la maladie (étude principale. (s.c : signifie sous cutanée).

Les traitement de l'étude sont : Secukinumab (AIN457) 300 mg et UVB à bande étroite.L’étude principale sera multicentrique, randomisée, à 2 groupes de traitement (secukinumab et UVB-be), groupes parallèles et ouverte.
Un total de 160 patients sera randomisé (assignés au hasard) dans le bras A1 ou le bras B1 dans environ 75 centres à travers le monde. Dans la mesure où un taux d’échec de sélection maximum de 20 % est attendu, environ 245 patients seront sélectionnés.
Bras A1: 80 patients vont recevoir 300 mg de secukinumab par voie sous cutanée.
Bras B1: 80 patients vont recevoir un traitement par nb-UVB.

Criteri per la partecipazione alla sperimentazione

Les patients éligibles à l’inclusion dans cette étude doivent satisfaire l’ensemble des critères suivants :
1. Capacité à comprendre et communiquer avec l’investigateur, volonté et capacité à respecter toutes les activités de l’étude et fourniture du consentement éclairé écrit, daté et signé (personnellement ou par un témoin) avant toute évaluation de l’étude
2. Âge : 18 à 40 ans inclus
3. Psoriasis en plaques d’apparition récente, dont les premières plaques sont apparues au cours des 12 mois précédant la randomisation et naïf de tout traitement systémique et de toute photothérapie (groupes A1, A2 et groupe B1)
4. Psoriasis en plaques chroniques dont les premiers symptômes ont commencé au moins 5 ans auparavant et intolérance ou réponse inadéquate à la photothérapie ou à tout traitement systémique, y compris les médicaments biologiques, sauf les inhibiteurs de l’IL-17 A (groupe C1 et groupe C2)
5. Psoriasis en plaques modéré à sévère défini à la sélection et à l’inclusion par un score PASI ≥ 10 et une surface corporelle (SC) atteinte ≥ 10 % et un score d’évaluation globale de l’investigateur (IGA mod 2011) ≥ 3

Les patients répondant à l’un des critères suivants ne seront pas éligibles à l’inclusion dans cette étude :
1. Formes de psoriasis autres que psoriasis en plaques (par exemple, pustuleux, érythrodermique, en gouttes, photosensible et induit par les médicaments)
2. Utilisation en cours de traitements interdits
3. Antécédents de traitement par photothérapie ou traitement systémique
4. Grossesse ou allaitement en cours
5. Femme en âge de procréer, définie comme physiologiquement apte à être enceinte, sauf si elle utilise un moyen efficace de contraception pendant la période de traitement ou plus longtemps si les données de prescription autorisées localement le requièrent (par exemple, 20 semaines dans l’UE)

Key Inclusion Criteria:
- Able to understand and communicate with the investigator, willing and capable to
comply with all study procedures, and provide written signed and dated informed
consent (personally or by a witness) before any assessment is performed.
- Aged 18 to 50 years inclusive.
- New-onset plaque psoriasis with appearance of the first psoriasis plaques within the
last 12 months before randomization and na?ve to any systemic treatment and
phototherapy (Arm A1, Arm A2, and Arm B1). Episodes of mild psoriasis, which occurred
at least 3 years before screening and resolved spontaneously within 6 months will be
accepted.
- Chronic plaque psoriasis with appearance of the first psoriasis symptoms 5 years or
longer and intolerance or inadequate response to phototherapy or any systemic
treatment including biologicals, except for IL-17A inhibitors (Arm C1 and Arm C2).
- Moderate to severe plaque psoriasis defined at screening and baseline by PASI >= 10,
and body surface area (BSA) >= 10%, and IGA mod 2011 >= 3.
Key Exclusion Criteria:
- Forms of psoriasis other than plaque-type (e.g., pustular, erythrodermic, guttate,
light sensitive, and drug induced).
- Ongoing use of prohibited treatments.
- Previous treatment with phototherapy or any systemic treatment.
- Pregnant or nursing (lactating) women.
- Women of childbearing potential, defined as all women physiologically capable of
becoming pregnant, unless they are using effective methods of contraception during the
Treatment period or longer if required by locally approved prescribing information
(e.g., 20 weeks in the EU and countries where applicable for secukinumab).
