Die Studie untersucht die Inzidenz von Herzrhythmusstörungen nach einer Standardbehandlung (Radiofrequenzablation des cavotrikuspidalen Isthmus) oder einer neuartigen Behandlung (Pulmonalvenenisolation mittels Cryoballoon) bei Patienten mit Vorhofflattern.
Cryoballon- und Radiofrequenzablation basieren auf dem gleichen Prinzip:
Das Herzmuskelgewebe, das die Rhythmusstörungen auslöst, wird mit Hitze (RFA) oder Kälte (Cryoablation) verödet.
Nach der Standardbehandlung (Radiofrequenzablation) entwickeln einige Patienten jedoch nochmals Vorhofflattern, oder sogar eine andere Form von Herzrhythmusstörungen und zwar Vorhofflimmern (AF).
Mit dieser Studie möchten wir Patienten in Gruppen Standardbehandlung (Radiofrequenzablation) versus neuartige Behandlung (Cryoablation) zuordnen und wiederauftretende Herzrhythmusstörungen vergleichen. Bei vorherigen Studienergebnissen haben Patienten mit einer Cryoablation deutlich weniger wiederauftretende Herzrhythmusstörungen als die Radiofrequenzablation.

Inzidenz von Vorhofflimmern und Vorhofflattern nach Ablation
Durch die Ablation des cavotrikuspidalen Isthmus wird Vorhofflattern sehr effektiv behandelt, 30% der Patienten entwickeln aber nach 2 Jahren neu Vorhofflimmern.
Da arrhythmogene Trigger in den Pulmonalvenen (PV) nicht nur Vorhofflimmern auslösen, sondern auch für Vorhofflattern verantwortlich zu sein scheinen, besteht die Hoffnung, dass durch die Isolation der PV mittels Cryoballon beide Rhythmusstörungen behandelt werden und die Häufigkeit des Wiederauftretens einer Rhythmusstörung abnimmt.

Criteri per la partecipazione alla sperimentazione

An der Studie können Personen teilnehmen,
• die mindestens 18 Jahre alt und jünger als 80 Jahre alt sind
• die dokumentiertes Vorhofflattern haben
• die bereit sind, eine schriftliche Einverständniserklärung zur verschlüsselten Freigabe ihrer Daten abzugeben.

· Personen, die zuvor dokumentierte Vorhofflimmern-Episoden von länger als 30 Sekunden
hatten
· Personen, die vorherige Ablationsbehandlungen hatten
· Personen, die nur ambulant ein Vorhofflattern diagnostiziert wurden
· Personen, die im EKG ein Vorhofflattern im linken Vorhof haben
· Personen, die ein dokumentiertes Vorhofflattern mit 1:1 AV-Überleitung und
hämodynamische Instabilität haben
· Personen, bei denen wegen eines implantierten PFO-Schirmchens oder anderen
anatonischen Gründen kein Zugang zum linken Vorhof möglich ist.
· Personen, deren linker Vorhof einen Durchmesser über 5.5 cm (Plax M-mode) hat
· Personen, die keine Indikation für eine Ablation haben
· Personen, die eine schwere Funktionseinschränkung der linken Hauptkammer
(LVEF<30%) haben
· Personen, die einen Schlaganfall innerhalb der letzten 3 Monaten erlitten haben
· Personen, die keine therapeutische Antikoagulation („Blutverdünnung“) nehmen dürfen
· Personen, die ein BMI über 40 (Übergewicht) haben
· Personen, die unter extreme Gebrechlichkeit leiden. Je nach Fall werden wir die
Gebrechlichkeit berechnen und unter 7 ausschliessen.
· Personen, die einen Mitralklappenersatz („künstliche Herzklappe“) haben
· Personen, die bereits einen Herzschrittmacher (ICD, CRT oder PM) erhalten haben
· Personen, die eine schwere Nierenfunktionsstörung haben (eGFR unter 30ml/ min)
· Personen, die vorherige Klappenersatzintervention oder prothetische Herzklappen hatten
· Schwangere oder stillende Frauen.
Frauen im gebärfähigen Alter müssen sich vor Beginn der Studienteilnahme Schwangerschaftstests unterziehen.
· Personen, die keine Implantation eines Herzmonitors möchten.

Inclusion Criteria:
- 1. Age 18-80 years
- 2. Patients referred for catheter ablation for typical atrial flutter. The atrial
flutter may be either persistent or paroxysmal, with at least one episode having been
documented on 12-lead ECG. In the view of the treating physician, the ECG morphology
should be compatible with a CTI-dependent circuit, either counterclockwise or
clockwise.
Exclusion Criteria:
- 1. Any evidence of previously documented atrial fibrillation
- 2. Previous cavo-tricuspid isthmus ablation or atrial fibrillation ablation
- 3. Atrial flutter documented solely on Ambulatory monitoring
- 4. Atrial flutter morphology on ECG suggestive of a left atrial flutter
- 5. History of atrial flutter with 1:1 atrioventricular conduction and haemodynamic
compromise
- 6. Indwelling atrial-septal defect occluder device, or any anatomical reason that
precludes left atrial access
- 7. Left atrial diameter (PLAX M-mode) >5.5 cm
- 8. Severe left ventricular dysfunction (LV ejection fraction < 30% on
Echocardiography)
- 9. Recent stroke/transient ischaemic attack within 3 months
- 10. Inability or unwillingness to take oral anticoagulant treatment
- 11. Morbid obesity (Body Mass Index =40)
- 12. Extreme frailty (A score of 7,8 or worse on the Clinical Frailty Scale)
- 13. Implanted metal prosthetic valve(s) in mitral position
- 14. Indwelling cardiac resynchronisation therapy device, pacemaker or implantable
cardioverter defibrillator
- 15. Advanced Renal dysfunction (eGFR<30 ml/min)
- 16. Pregnancy
- 17. Severe valvular heart disease of any kind as assessed by the investigator
- 18. Previous valve replacement surgery or other prosthetic heart valve
