Die Studie sammelt Daten der neuen Ilivia ICD Familie (= Implantierbare Kardiale Defibrillatoren) und der neuen Plexa ICD Elektrode (=Elektrode, die in die rechte Herzkammer implantiert wird und dort eine Therapie (elektrischen Shock) abgibt). Diese Daten werden von der benannten Stelle (= Organistaion, welche die Geräte prüft und die CE Marke vergibt) gefordert um eventuelle Risiken aufzudecken, die weiterhin eventuell bestehen obwohl die Geräte die CE Marke tragen. Weiterhin werden die erhobenen Daten auch genutzt um die Zulassung der Geräte in Ländern zu unterstützen, in denen die CE Marke keine Gültigkeit hat.
Patienten, die an dieser Studie teilnehmen werden enstprechend der klinischen Routine mit einem ICD implantiert und dann für 6 Monate nachverfolgt. Dabei werden Daten zum Zeitpunkt der Implantation, der Krankenhausentlassung, 3 Monate nach der Implantation und 6 Monate nach Implantation sowie evt. zusätzlichen nicht geplanten Besuchen in diesem Zeitraum, aufgenommen. Außerdem werden während der gesamten Zeit alle auftretenden medizinischen Ereignisse des Patienten gesammelt und dokumentiert.

Untersucht werden Patienten die eine Gefährdung bezüglich schneller unregelmäßiger Herzschläge haben (ventriculäre Arrhythmien) und damit potentiell ein Risiko haben an plötzlichem Herztod zu sterben. Solche Patienten haben eine medizinischen Indikation zur Implantation eines Implantierbaren Kardialen Defibrillators (ICD). Wenn der Patient gleichzeitig auch noch an Herzinsuffizienz (= Herzversagen) leidet, wird ein ICD implantiert der gleichzeitig auch die Wiederherstellung der gleichmäßigen Muskelkontraktion des Herzens ermöglicht (CRT-D = Kardiale Resynchronisationstherapie mit Defibrillator). In der Studie erhalten die Patienten eine neue Gerätegeneration implantiert (Ilivia ICD Familie und Plexa ICD Elektrode).

In der Studie werden für beide Gerätetypen Standarddaten erhoben, wie z.B. Einschätzungen des Arztes bezüglich der Handhabung der Geräte oder Gerätemesswerte, wie sie bei jeder Routinekontrolle von ICDs auch außerhalb von Studien erhoben werden.
Zusätzlich zu diesen Standarddaten werden bei Patienten, die einen CRT-D der Ilivia Familie erhalten noch zusätzliche Messungen zu einer neuen Funktion des Gerätes gemacht.
Außerdem werden bestimmte Geräteeinstellungen vorgegeben.

- Kontraindikation für ICD beziehungsweise CRT-D Therapie
- Lebenserwartung unter 6 Monaten
- Teilnahme an einer anderen interventionellen klinischen Studie
Inclusion/Exclusion Criteria

Inclusion Criteria:
- Standard indication for ICD or CRT-D therapy according to clinical practice
- De novo implantation or upgrade/exchange (group A only) from existing ICD, CRT-D or
pacemaker implant
- Patient is able to understand the nature of the clinical investigation and provides
written informed consent
- Patient is able and willing to complete all routine study visits at the
investigational site
- Patient accepts Home Monitoring concept
- Age = 18 years
Exclusion Criteria:
- Contraindication to ICD or CRT-D therapy, respectively
- For CRT-D patients in group A only: physician not willing to activate MultiPole Pacing
in the patient
- Cardiac surgical procedure planned within 6 months after implantation (including also
interventional procedures like ablation, valve replacement etc.). Procedures to occur
during or prior to implantation are not exclusionary.
- Expected to receive heart transplant or ventricular assist device within 6 months
- Life expectancy less than 6 months
- Participation in any other interventional clinical investigation
- Pregnant or breastfeeding at time of enrollment
