Das Schädel-Hirn-Trauma (SHT) gehört zu den führenden Todesursachen und ist häufig Ursache bleibender Behinderung. Eine erfolgsversprechende Therapie des SHT ist die Behandlung mittels Hypothermie (Kühlung). Diese Therapie wird erfolgreich zur Behandlung des Hirnschadens nach Herzstillstand angewendet. Es gibt klinische Versuche mit Hinweisen, dass die Hypothermiebehandlung wirksam sein könnte. Aber bis heute stehen die Belege aus, dass mit Sicherheit gesagt werden könne, alle Patienten mit schwerem SHT sollten mittels Hypo-thermie behandelt werden, und wie mit allen Therapien sind Risiken und Nebenwirkungen verbunden.
Die POLAR Studie wurde mit der Fragestellung ob frühe Hypothermiebehandlung bei Patienten mit schwerem SHT zu einem besseren Verlauf führt, entwickelt.
Bewusstlose Patienten werden bezüglich der Studienkriterien beurteilt, und wenn sich die Patienten für die Studie eignen und es für die Patienten sicher ist, sie zu kühlen, werden sie zufällig (ähnlich des Werfen einer Münze) in 2 Gruppen eingeteilt. Die Gruppe, welche der Hypothermie zugeteilt wurden, bekommen zusätzlich zur Standardtherapie gekühlte Infusionen verabreicht. In Bern wird die Behandlung auf der Notfallstation begonnen. Im weiteren Verlauf werden die Patienten der Hypothermiegruppe für 3 Tage auf 33°C gekühlt, dies mittels der Therapie analog zu den Patienten mit Herz-Kreislauf-Stillstand (Kühlkatheter/Kühldecken).
Die Patienten der Kontrollgruppe hat die normale Körpertemperatur von 37°C zum Ziel, im Falle von Fieber werden diese Patienten ebenfalls auf 37°C gekühlt (Kühlkatheter/Kühldecke). Nach 3 Tagen werden die Patienten in der Hypothermiegruppe langsam wieder auf Normaltemperatur erwärmt. Falls beim Wiedererwärmen Probleme mit erhöhtem Hirndruck durch Schwellung des traumatisierten Gehirns entstehen, wird der Patient erneut gekühlt. 6 Monate nach Unfall wird die Erholung/Genesung evaluiert.

• Stumpfes Trauma mit der klinischen Diagnose eines schweren Schädel-Hirn-Traumas mit einem GCS < 9
• Geschätzes oder bekanntes Alter grösser/gleich18 und kleiner 60 Jahre
• Patient ist intubiert bzw. die Intubation steht unmittelbar bevor

• Klinisch erhebliche Blutung
• GCS 3 (= tiefes Koma) und areaktive Pupillen (Pupillen reagieren nicht auf Licht)

Inclusion Criteria:
- Blunt trauma with clinical diagnosis of severe TBI and GCS <9
- Estimated age = 18 and < 60 years of age
- The patient is intubated or intubation is imminent
Exclusion Criteria:
- Pre-hospital:
- Clinical diagnosis of drug or alcohol intoxication as predominant cause of coma
- Randomisation unable to be performed within 3 hrs of estimated time of injury
- Estimated transport time to study hospital >2.5hrs
- Able to be intubated without drugs
- Systolic BP <90mmHg
- Heart rate > 120bpm
- GCS=3 + un-reactive pupils
- Penetrating neck/torso injury
- Known or obvious pregnancy
- Receiving hospital is not a study site
- Evidence of current anti-coagulant treatment
- Emergency Dept:
- Clinical diagnosis of drug or alcohol intoxication as predominant cause of coma
- Randomisation unable to be performed within 3 hrs of estimated time of injury
- Able to be intubated without drugs
- GCS=3 + un-reactive pupils
- Persistent Systolic BP <90mmHg
- Clinically significant bleeding likely to require haemostatic intervention, for
example:
- Bleeding into the chest, abdomen or retro-peritoneum likely to require
surgery +/- embolisation
- Pelvic fracture likely to require surgery +/- embolisation
- More than two long bone fractures requiring operative fixation
- Penetrating neck/torso injury
- Positive urine or blood pregnancy test
- Evidence of current anti-coagulant treatment
- In the treating clinician's opinion, "cooling" is not in the patient's best
interest
